摘 要:目的观察两种药物治疗绝经期骨质疏松症的疗效及对骨转换标志物的影响比较。方法随机将2017年1月至2018年2月本院骨科收治的120例绝经期骨质疏松症患者分为观察组与对照组,各60例,分别接受阿法骨化醇、雷奈酸锶口服治疗。观察两组治疗前后不同时间点的腰椎、股骨颈的骨密度(BMD)和视觉模拟法(VAS)评分、Ⅰ型原胶原氨基端肽(PINP)、Ⅰ型胶原羧基端肽交联(β-CTX)的含量。结果与治疗前比较,治疗6、12个月后两组患者的腰椎和股骨颈的骨密度均呈上升趋势,VAS评分均呈下降趋势,且观察组疗效优于对照组,差异具有统计学意义(P<0.05)。与治疗前相比,治疗6、12个月后观察组PINP浓度呈上升趋势,而对照组PINP浓度均呈下降趋势;两组患者的β-CTX浓度均呈下降趋势,且观察组显著低于对照组,差异具有统计学意义(P<0.05)。结论阿法骨化醇和雷奈酸锶均能使绝经期骨质疏松症的骨密度上升,疼痛程度下降,其中雷奈酸锶、对骨转换标志物的影响优于阿法骨化醇。Objective To observe the curative effect of two drugs on menopausal osteoporosis and the influences on bone turnover markers.Methods A total of 120 patients with menopausal osteoporosis who were admitted to orthopedics department in our hospital from January 2017 to February 2018 were randomLy divided into observation group and control group, with 60 cases in each group. The two groups were given oral alfacalcidol and strontium ranelate, respectively. The bone mineral density(BMD) of lumbar spine and femoral neck, score of visual analogue scale(VAS), contents of procollagen type Ⅰ N-terminal propeptide(PINP) and β cross-linked C-telopeptide of type Ⅰcollagen(β-CTX) in both groups at different time points before and after treatment were observed. Results Compared to before treatment, BMD of lumbar spine and femoral neck in both groups at 6 and 12 months after treatment was increased, while VAS score was decreased. The curative effect of observation group was better than that of control group(P<0.05). Compared with those before treatment, concentration of PINP in observation group was increased at 6 and 12 months after treatment, while concentration of PINP in control group was decreased. The concentration of β-CTX in both groups was decreased. The abode index in observation group was significantly lower than that in control group(P<0.05). Conclusion Both alfacalcidol and strontium ranelate increase BMD of menopausal osteoporosis, and decrease pain degree. The curative effect of strontium ranelate and its influences on bone turnover markers was better than those of alfacalcidol.
