摘　　要：目的探究CT胸部扫描对肺结核的诊断价值。方法选取本院2016年2月至2018年10月疑诊为肺结核的患者65例,对其进行常规X线检查和CT胸部扫描,以肺结核病原学及病理学检查结果为标准,比较两种检查方法的诊断准确性、敏感性和特异性;然后再对两组检查图像进行分析,对比两种检查方式对肺结核患者各种典型征象的检出率。结果肺结核48例,非肺结核17例,CT胸部扫描在肺结核诊断中的敏感性、特异性和准确性大于常规X线检查结果,差异有统计学意义(P<0.05);分析两种检查方法的征象显示情况,在48例肺结核患者中,CT胸部扫描对树芽征、空洞、肺门或纵膈淋巴结增大以及支气管血管周围间质增厚征象的检出率高于常规X线检查,差异有统计学意义(P<0.05);另一方面,在磨玻璃密度与实变、胸水、肺内球形结核以及随机分布结节等征象的检出率上,两种检查方法比较差异无统计学意义。结论 CT胸部扫描能够有效检查出肺结核,并对其典型征象具有较高的检出率,能够为临床诊治提供有效的参考价值,在临床上值得进一步推广和使用。Objective To explore the diagnostic value of CT chest scan in pulmonary tuberculosis. Methods 65 patients suspected of pulmonary tuberculosis from February 2016 to October 2018 were examined by routine X-ray examination and CT chest scan respectively. According to the results of etiological and pathological examination of pulmonary tuberculosis, the diagnostic accuracy, sensitivity and specificity of the two methods were compared, and then the images of the two groups were analyzed and the detection rates of various typical signs of pulmonary tuberculosis patients were compared between the two methods. Results There were 48 cases of pulmonary tuberculosis and 17 cases of non-pulmonary tuberculosis. The sensitivity, specificity and accuracy of CT chest scan in the diagnosis of pulmonary tuberculosis were significantly higher than those in the routine X-ray examination(P<0.05). The signs of the two methods were analyzed. In 48 patients with pulmonary tuberculosis, the detection rate of tree bud sign, cavity, enlarged hilar or mediastinal lymph nodes and thickening of stroma around bronchial vessels by CT chest scan was significantly higher than that by conventional X-ray examination(P<0.05). On the other hand, there was no significant difference between the two methods in the detection rate of ground glass density and consolidation, hydrothorax, pulmonary spherical tuberculosis and randomLy distributed nodules.Conclusion CT chest scan can effectively detect pulmonary tuberculosis, and has a high detection rate for its typical signs, which can provide effective reference value for clinical diagnosis and treatment, and is worthy of further promotion and use in clinic.
