药物性肝损伤针对性治疗的回顾性分析

摘　　要：目的:比较不同类型药物性肝损伤使用不同保肝药物进行针对性治疗的效果。方法:回顾性分析某院2013年1月-2017年12月诊断为药物性肝损伤的病历,收集病历信息,统计不同保肝药物治疗不同类型药物性肝损伤的疗程、费用及疗效。结果:对于肝细胞损伤型肝损伤的治疗,异甘草酸镁注射液治疗疗程最短[(5.47±1.94)d,P<0.05],总有效率最高(95.5%,P<0.05),多烯磷脂酰胆碱注射液治疗费用最低[(664.62±273.05)元,P<0.05];对于胆汁淤积型肝损伤,注射用丁二磺酸腺苷蛋氨酸治疗疗程最短[(5.38±2.02)d,P<0.05],费用最低[(575.70±215.56)元,P<0.05],总有效率最高(91.1%,P<0.05);对于混合型肝损伤,3种保肝药物治疗疗程及总有效率均无显著性差异(P>0.05),但多烯磷脂酰胆碱注射液治疗费用较低[(611.35±206.47)元,P<0.05]。结论:对于不同类型的肝损伤,针对性的治疗能缩短治疗疗程,降低平均治疗费用,提高保肝药物的治疗效果。OBJECTIVE To compare the effects of different hepatoprotective drugs in the targeted treatment of different types of drug-induced liver injury. METHODS The medical records of drug-induced liver injury diagnosed in our hospital from January 2013 to December 2017 were retrospectively analyzed. The medical records were collected and the course, cost and efficacy of different hepatoprotective drugs in the treatment of different types of drug-induced liver injury were statistically analyzed. RESULTS For the treatment of hepatocellular injury, Magnesium Isoglycyrrhizinate Injection had the shortest course of treatment[(5.47±1.94)d, P<0.05], while the total effective rate was the highest(95.5%, P<0.05). The average treatment cost of Polyene phosphatidylcholine injection was the lowest [(664.62±273.05)yuan, P<0.05]. For cholestatic liver injury, the treatment course of adenosine succinate for injection was the shortest [(5.38±2.02)d, P<0.05], and the average cost was the lowest [(575.70±215.56) yuan,P<0.05]while the total effective rate was the highest(91.1%, P<0.05);for mixed liver injury, there was no significant difference in the treatment duration and total effective rate among the three hepatoprotective drugs(P>0.05), but the treatment costs of Polyene phosphatidylcholine injection were lower than others [611.35±206.47)yuan, P<0.05]. CONCLUSION For different types of liver injury, targeted treatment can shorten the course of treatment, reduce the average treatment cost and improve the therapeutic effect of hepatoprotective drugs.
