CT双能量成像诊断痛风患者的价值研究

摘　　要：目的评价分析CT双能量成像用于痛风诊断的临床价值,并以此指导后期临床治疗。方法方便选取该院自2017年1月—2018年12月收治并经临床检查确诊的45例痛风患者纳入该次研究为观察组,另取同期健康体检者45名作为对照组,两组患者均接受双源CT双能量成像技术检查,对两组患者尿酸结晶阳性率进行统计分析,并分析双源CT双能量成像技术的敏感度、特异度、准确率。结果痛风组患者尿酸盐结晶阳性率为84.44%,对照组尿酸盐结晶阳性率为11.11%,痛风组较对照组高(χ2=8.783,P=0.021);观察组患者尿酸盐沉积在手、腕处者所占比例为26.67%,尿酸盐沉积在足、踝处者所占比例为42.22%,两组差异有统计学意义(P<0.05)。结论采用双源CT双能量成像技术对痛风性关节炎的诊断准确性较高,可用于痛风早期筛查诊断,并为临床治疗提供指导依据,具有较高的临床应用价值。Objective To evaluate the clinical value of CT dual-energy imaging in the diagnosis of gout,and to guide the clinical treatment.Methods Convenient selected 45 gout patients admitted to the hospital from January 2017 to December 2018 and confirmed by clinical examination were included in the observation group.Another 45 healthy subjects were selected as the control group.Both groups received double source CT dual-energy imaging technique was used to analyze the positive rate of uric acid crystallization in two groups of patients,and the sensitivity,specificity and accuracy of dual-source CT dual-energy imaging technique were analyzed.Results The positive rate of urate crystallization in the gout group was 26.67%,and the positive rate of urate crystallization in the control group was 11.11%.The gout group was higher than the control group(χ2=8.783,P=0.021).The urate deposition proportion of hands and wrists in the observation group was 4.44%,and the proportion of urate deposits in the feet and sputum was 42.22%.The difference was statistically significantbetween the two groups(P<0.05).Conclusion The dual-source CT dual-energy imaging technique has high diagnostic accuracy for gouty arthritis.It can be used for early screening and diagnosis of gout,and provides guidance for clinical treatment.It has high clinical application value.
