研究探讨胸腔镜肺大泡手术麻醉中双腔支气管插管的临床应用效果与分析

摘　　要：目的分析双腔支气管插管在胸腔镜肺大泡手术麻醉中的应用效果。方法方便选择该院2016年1月—2018年12月中60例需要接受胸腔镜肺大泡手术治疗的患者,包括51例男患者、9例女患者,对患者临床资料进行回顾性总结,分析麻醉中双腔支气管插管的应用价值。结果该组60例患者双腔支气管插管一次成功率86.67%,术后没有患者出现并发症;插管前、插管后25 min及50 min患者血氧饱和度、心率、平均动脉压、PaCO2没有明显变化(P>0.05);PaO2在插管后25 min出现明显下降,到插管后50 min有回升,与插管前差异有统计学意义(F=4.585,P<0.05),Ppeak在插管后25 min、50 min均有升高,与插管前差异有统计学意义(F=3.625,P<0.05)。结论胸腔镜肺大泡手术麻醉中应用双腔支气管插管能够获得更好的麻醉效果,减小体征指标波动,保证手术顺利进行,提升手术质量。Objective To analyze the effect of double lumen bronchial intubation in thoracoscopic alveolar anesthesia.Methods convenient select sixty patients who underwent thoracoscopic pulmonary bullae surgery in the hospital from January 2016 to December 2018 were enrolled,including 51 male patients and 9 female patients.The clinical data of the patients were retrospectively analyzed of the value of double lumen bronchial intubation in anesthesia.Results The success rate of double lumen bronchial intubation in this group of patients was 86.67%.No complications occurred after operation.Blood oxygen saturation,heart rate,mean arterial pressure,blood pressure saturation,heart rate,mean arterial pressure,intubation,and there was no statistically significant change in 25 min and 50 min after intubation PaCO2(P>0.05);PaO2 showed a significant decrease at 25 minafter intubation,and rebounded 50 minutes after intubation,and the difference was statistically significant before intubation(F=4.585,P<0.05).There was an increase in 25 min and 50 min after Ppeak intubation,and the difference was statistically significant before intubation(F=3.625,P<0.05).Conclusion Double-lumen bronchial intubation in thoracoscopic alveolar sac anesthesia can obtain better anesthesia effect,reduce the fluctuation of physical signs,ensure the smooth operation and improve the quality of surgery.
