探讨多种血液净化方法联用并结合护理干预在尿毒症CKD-MBD治疗中的临床效果

摘　　要：目的探讨多种血液净化方法联用并结合护理干预在尿毒症CKD-MBD治疗中的临床效果。方法方便选取2017年1月—2018年12月在该院接受治疗的52例尿毒症患者作为该次实验的研究对象,随机分为两组,每组26例。对照组采用血液透析方法(HD)治疗及干预手段,观察组采用多种血液净化结合护理干预,即血液透析与血液透析滤过(HD+HDF)联合治疗。对比分析两组患者的临床效果情况。结果观察组(第2、4阶段)PTH[(338.54±148.21)、(289.02±147.23)pg/mL]及血磷[(1.92±0.51)、(1.52±0.42)mmol/L]指标改善方面,明显优于对照组[(469.85±150.47)、(425.25±149.52)pg/mL,(2.49±0.78)、(2.21±0.65)mmol/L](t=13.891、16.654、5.701、8.625,P<0.05);且该组患者的临床症状[血压:(14.52±2.11)kPa;骨痛:1例;营养不良:2例;胸闷心慌:1例;皮肤瘙痒:0例]也明显优于对照组[血压:(20.12±2.21)kPa;骨痛:4例;营养不良:7例;胸闷心慌:5例;皮肤瘙痒:3例](t=13.659,χ2=0.996、1.680、1.507、0.979,P<0.05)。结论采用多种血液净化方法联用并结合护理干预对尿毒症CKD-MBD临床效果显著,且能有效改善患者治疗后的临床症状,值得进一步应用推广。Objective To explore the clinical effects of multiple blood purification methods combined with nursing intervention in the treatment of uremia CKD-MBD.Methods A total of 52 uremic patients who were treated in the hospital from January 2017 to December 2018 were convenient selected and enrolled in this study.They were randomly divided into two groups,26 cases in each group.The control group was treated with hemodialysis(HD)and intervention.The observation group used a variety of blood purification combined with nursing intervention,namely hemodialysis combined with hemodiafiltration(HD+HDF).The clinical effects of the two groups of patients were compared and analyzed.Results In the observation group(stages 2 and 4),the improvement of PTH[(338.54±148.21),(289.02±147.23)pg/mL]and blood phosphorus[(1.92±0.51),(1.52±0.42)mmol/L]was significantly better than that of the control group[(469.85±150.47),(425.25±149.52)pg/mL,(2.49±0.78),(2.21±0.65)mmol/L](t=13.891,16.654,5.701,8.625,P<0.05);and the clinical symptoms of this group of patients[blood pressure:(14.52±2.11)kPa;bone pain:1 cases;malnutrition:2 cases;chest tightness palpitation:1 cases;skin itching:0 cases],also significantly better than the control group[blood pressure:(20.12±2.21)kPa;bone pain:4 cases;malnutrition:7 cases;chest tightness palpitation:5 cases;skin itching:3 cases](t=13.659,χ2=0.996,1.680,1.507,0.979,P<0.05).Conclusion The combination of multiple blood purification methods combined with nursing intervention has significant clinical effects on uremia CKD-MBD,and can effectively improve the clinical symptoms of patients after treatment,which is worthy of further application.
