超声刀行小切口甲状腺手术中暴露喉返神经的临床疗效分析

摘　　要：目的对甲状腺手术患者在采用超声刀行小切口来暴露喉返神经后的临床治疗效果进行探究。方法 160例采用超声刀行小切口甲状腺手术治疗的患者,随机分为实验组和对照组,各80例。在术中甲状腺下动脉的处理时,实验组暴露喉返神经,对照组未暴露喉返神经。观察比较两组患者术后喉返神经损伤率及其3个月后恢复情况。结果实验组患者喉返神经损伤率1.25%明显低于对照组的8.75%,差异具有统计学意义(P<0.05)。随访3个月后,实验组喉返神经损伤患者完全恢复率为100.00%(1/1),永久性损伤率为0;对照组喉返神经损伤患者完全恢复率为71.43%(5/7),永久性损伤率为28.57%(2/7)。结论在采用超声刀进行小切口甲状腺手术时,暴露喉返神经进行手术有助于减少超声刀对喉返神经的损伤。Objective To investigate the clinical effect of exposure of recurrent laryngeal nerve in small incision thyroid surgery with ultrasonic knife. Methods A total of 160 cases of thyroid surgery patients with ultrasonic knife were randomly divided into experimental group and control group, with 80 cases in each group. During the operation of the inferior thyroid artery, the recurrent laryngeal nerve was exposed in the experimental group, and the recurrent laryngeal nerve in the control group was not exposed. The recurrent laryngeal nerve injury rate after operation and recovery situation after 3 months of treatment was observed and compared between the two groups. Results After operation, the recurrent laryngeal nerve injury rate 1.25% in the experimental group was obviously lower than 8.75% in the control group, and the difference was statistically significant(P<0.05). After 3 months of follow-up, the complete recovery rate in recurrent laryngeal nerve injury patients was 100.00%(1/1) and permanent injury rate was 0. The complete recovery rate in recurrent laryngeal nerve injury patients was 71.43%(5/7), and permanent injury rate was 28.57%(2/7). Conclusion During the small incision thyroid surgery with ultrasonic knife, exposing the recurrent laryngeal nerve helps to reduce the injury of the recurrent nerve by the ultrasonic knife.
