摘  要:目的探讨颈动脉斑块超声造影特征及血清超敏C反应蛋白(hs-CRP)水平与脑梗死的关系。方法选取我院有颈动脉斑块的住院患者70例分成脑梗死组(32例)和非脑梗死组(38例),行颈动脉斑块超声造影检查,将58例有增强患者分成脑梗死增强组(30例)和非脑梗死增强组(28例),比较两组间造影斑块面积(A)、峰值强度(PI)、增强密度(ED)、血清hs-CRP水平差异并分析与脑梗死的关系。结果脑梗死增强组PI、ED均高于非脑梗死增强组(P分别为0.030、0.009)。脑梗死增强组血清hs-CRP水平高于非脑梗死增强组(P<0.001)。脑梗死增强组斑块A与非脑梗死增强组的差异无统计学意义(P=0.538)。脑梗死增强组ED与血清hs-CRP水平呈正相关(r=0.723,P<0.001)。结论颈动脉斑块内新生血管形成及血清hs-CRP水平与脑梗死的发生相关,检测二者有助于评估斑块的稳定性,预测脑血管事件。Objective To investigate the relationship between features of carotid plaque contrast-enhanced ultrasonography(CEUS)and serum high-sensitivity C-reactive protein(hs-CRP)levels and cerebral infarction.Methods Seventy patients with carotid plaques in our hospital were enrolled in the cerebral infarction group(n=32)and the non-cerebral infarction group(n=38).Carotid plaques were examined by contrast-enhanced ultrasound.58 patients with plaque CEUS were divided into cerebral infarction enhancement group(n=30)and non-cerebral infarction enhancement group(n=28).The examined plaque area(A),the peak intensity(PI),enhanced density(ED)of plaques and serum hs-CRP levels in the two groups were compared and analyzed for their relationship with cerebral infarction.Results PI and ED in the cerebral infarction-enhanced group were higher than those in the non-cerebral infarction-enhanced group(P=0.030,0.009).Serum hs-CRP levels were higher in the cerebral infarction-enhanced group than in the non-cerebral infarction-enhanced group(P<0.001).There was no significant difference in plaque A between the cerebral infarction-enhanced group and the non-cerebral infarction-enhanced group(P=0.538).There was a positive correlation between ED and serum hs-CRP levels in the cerebral infarction-enhanced group(r=0.723,P<0.001).Conclusions Neovascularization and serum hs-CRP levels in carotid plaques were associated with the occurrence of cerebral infarction.Both of them were helpful to evaluate the stability of plaque and predict cerebrovascular events.
