摘  要:目的探讨超声造影(CEUS)联合动态血流成像技术(ADF)对肾动脉狭窄(RAS)的诊断价值。方法回顾性分析我院高血压病患者75例、150条肾动脉,采用CEUS联合ADF技术观察其双侧肾动脉有效管腔内径及血流充盈情况,测量相应血流动力学指标,判断肾动脉狭窄程度。以DSA为诊断金标准,分别计算常规彩色多普勒超声成像(CDFI)及CEUS联合ADF诊断肾动脉狭窄的灵敏度、特异度、阳性预测值、阴性预测值、准确度。结果CDFI对肾动脉狭窄诊断阳性预测值73.07%、阴性预测值87.90%、准确度85.33%、灵敏度55.88%、特异度93.96%;CEUS联合ADF对肾动脉狭窄诊断阳性预测值87.50%、阴性预测值94.91%、准确度93.33%、灵敏度82.35%、特异度96.55%。CDFI检查与CEUS联合ADF对肾动脉狭窄诊断相比,在阴性预测值与特异度上,两者差异没有统计学意义(P>0.05);在阳性预测值、准确度、灵敏度上后者均明显高于前者(P<0.05)。结论CEUS联合ADF技术对于肾动脉狭窄的诊断具有重要的临床价值。Objective To investigate the diagnostic value of contrast-enhanced ultra ultrasound(CEUS)combined with advanced dynamic flow imaging(ADF)in renal artery stenosis.Methods A retrospective analysis was made on 75 patients with hypertension from in our hospital.Contrast-enhanced ultrasound combined with advanced dynamic blood flow imaging technology was used to observe the two-dimensional lumen and blood flow filling of bilateral renal arteries.Hemodynamic indexes were measured to judge the degree of renal artery stenosis,With DSA as the gold standard,the sensitivity,specificity,positive predictive value,negative predictive value and accuracy of conventional color Doppler flow imaging(CDFI)and CEUS combined with ADF in diagnosing renal artery stenosis were calculated respectively.Results Conventional CDFI had positive predictive value 73.07%,negative predictive value 87.90%,accuracy 85.33%,sensitivity 55.88%,specificity 93.96%for diagnosis of renal artery stenosis.CEUS combined with ADF had positive predictive value 87.50%,negative predictive value 94.91%,accuracy 93.33%,sensitivity 82.35%,specificity 96.55%for diagnosis of renal artery stenosis.Compared with CEUS combined with ADF in diagnosis of renal artery stenosis,there was no significant difference in negative predictive value and specificity between conventional CDFI and CEUS combined with ADF(P>0.05).The latter was significantly higher than the former in positive predictive value,accuracy and sensitivity(P<0.05).Conclusions CEUS combined with ADF technology had important clinical value in the diagnosis of renal artery stenosis.
