多普勒联合二维超声检测右心血流对胎儿生长受限的意义

摘　　要：目的分析多普勒超声联合二维超声检测右心血流的效果,并评价其对胎儿生长受限(FGR)的诊治意义。方法选取我院孕期超声诊断为FGR孕妇138例为研究组;配对选取同时期正常妊娠的孕妇138例为对照组。所有孕妇行产前多普勒超声联合二维超声检测。结果(1)研究组胎儿的上、下腔静脉频谱测值均随孕周的增加而加大;(2)研究组胎儿的三尖瓣瓣口的频谱测值均明显低于同期对照组;(3)研究组不良妊娠结局例数均明显高于对照组。结论使用多普勒超声联合二维超声可有效了解胎儿右心的实时血流动力学改变,对围产期优生优育具有重要意义。Objective To analyze the effect of Doppler echocardiography combined with two-dimensional echocardiography in detecting right ventricular blood flow and to evaluate its diagnostic and therapeutic significance for fetal growth restriction(FGR).Methods 138 pregnant women with FGR diagnosed by ultrasound during pregnancy in our hospital from were selected as the study group,and 138 pregnant women with normal pregnancy during the same period were selected as the control group.All pregnant women underwent prenatal Doppler ultrasound combined with twodimensional ultrasound detection.Results(1)The frequency spectrum of superior and inferior vena cava in the study group increased with the increase of gestational weeks.(2)The frequency spectrum of tricuspid valve orifice in the study group was significantly lower than that in the control group at the same time.(3)The number of adverse pregnancy outcomes in the study group was significantly higher than that in the control group.Conclusions Doppler ultrasound combined with two-dimensional ultrasound can effectively understand the real-time hemodynamic changes of fetal right heart,which is of great significance for perinatal eugenics and fertility.
