王旭教授辨治糖尿病瘙痒症的临床经验

摘　　要：[目的]介绍王旭教授诊治糖尿病瘙痒症的临床经验。[方法]通过跟师学习、分析研究王旭教授临床治疗糖尿病瘙痒症的临床医案,从发病机制、治则治法、经验用药等方面总结王旭教授治疗糖尿病瘙痒症的学术思想及临证经验,并列举验案佐证。[结果]王旭教授认为糖尿病瘙痒症的发病机制以阴虚内热为本,络热血瘀为标,治当养阴清热、凉血化瘀;同时强调"瘀热"为本病的主要病理因素,清热凉血、化瘀通络当贯穿治疗始终。王教授还主张瘙痒症的辨治应始终在糖尿病整体调治的基础上进行,同时应嘱患者注意生活调养,调畅情志,合理饮食,适度锻炼,保持局部皮肤滋润。所举验案辨证为阴虚风燥、瘀热互结证,治以滋阴清热、化瘀通络,予养阴凉血化瘀汤加减,取得了较好的效果。[结论]王旭教授运用养阴清热、凉血化瘀法治疗糖尿病瘙痒症,临床疗效显著,值得学习和推广应用。[Objective] To introduce Professor WANG Xu’s clinical experience in diagnosis and treatment of diabetic pruritus. [Methods] By learning from teachers, analyzing and studying the clinical medical cases, the academic thoughts and clinical experience of Professor WANG Xu in treatment of diabetic pruritus were summarized from the aspects of pathogenesis, treatment principles and methods, experience medication, and one empirical case was presented for evidence. [Results] Professor WANG proposes that the pathogenesis of diabetes pruritus is based on Yin deficiency and internal heat, with collateral heat and blood stasis as the syndrome, and takes nourishing Yin and clearing heat, cooling blood and removing stasis as treatment principles.Professor WANG emphasizes that "stasis and heat" is the main pathological factor of the disease, treatment of clearing heat and cooling blood,removing blood stasis and clearing collaterals should run through the treatment process. Professor WANG suggests that the diagnosis and treatment of pruritus should always be based on the whole treatment of diabetes,at the same time, patients should be instructed to pay attention to life maintenance, emotional adjustment, reasonable diet, moderate exercise, maintain local skin moisture. The case was differentiated as dryness due to Yin deficiency, stagnation of blood stasis and heat, and achieved good results after treated with modified Yangyin Liangxue Huayu decoction according to the principle of nourishing Yin, clearing heat and removing stasis, dredging collaterals.[Conclusion] Professor WANG Xu takes nourishing Yin and clearing heat, cooling blood and removing blood stasis as main treatment methods in treating diabetic pruritus and achieves significant results. Professor WANG ’s experience in diagnosis and treatment of diabetic pruritus is worth learning and popularizing.
