摘  要:目的:探讨发作性睡病的夜间睡眠结构特点。方法:研究对象为2007年1月至2017年12月在福建省级机关医院神经内科门诊就诊的儿童患者,其中11例诊断为1型发作性睡病,20例为2型发作性睡病。同期收集20例有日间嗜睡主诉但排除相关睡眠疾病的儿童患者。使用多项睡眠监测仪进行整夜(>7h)连续包括16道脑电图的视频多项睡眠监测(video-polysomnography,PSG)及次日进行5次多次小睡潜伏期试验。结果:与对照组比较,2型发作性睡病人睡潜伏期缩短[(7.92±5.73)min vs(20.62±20.89)min,P<0.05],非快速眼球运动(non rapid eye movement,NREM)睡眠1期比例(N1%)增多(14.53±7.13vs10.12±5.63,P<0.05),差异有统计学意义。与1型发作性睡病比较,2型发作性睡病的入睡后觉醒时间(wake time after sleep onset,WASO)占总卧床时间的百分比(WASO%)减少(7.79±8.62vs18.15±10.19,P<0.05),快速眼球运动(rapid eye movement,REM)睡眠潜伏期延长,差异有统计学意义[(149.92±67.01)rain vs(93.50±106.61)min,P<0.05]。结论:发作性睡病患儿存在夜间睡眠结构紊乱。Objective:To explore the characteristics of night sleep structure in narcolepsy.Methods:The enrolled subjects were children patients who attending neurology clinic of Fujian Provincial Governmental H ospital from January 2007 to December 2017.Among the patients,11 cases were Type 1 narcolepsy,20 cases were Type 2 narcolepsy.In the same period,20 children with daytime sleepiness complaints,excluding sleep-related diseases,were collected.All subjects underwent overnight(﹥7 h)consecutive video-polysom nography(vPSG)with 16 channel EEG and m ultiple sleep latency test(MSLT)including five nap opportunities the next day.Results:Compared with the control group,the sleep latency of T ype 2 narcolepsy was shortened(7.92 min±5.73 min vs 20.62 min±20.89 min,P<0.05),and more non rapid eye movement(NREM)in sleep period 1(N 1%)(14.53土7.13 vs 10.12±5.63,P<0.05),and the difference was statistically significant.Compared with Type 1 narcolepsy,Tpye 2 narcolepsy had less wake time after sleep onset(WASO)in percentage of time in bed(WASO%)(7.79±8.62 vs 18.15±10.19,P<0.05)and longer rapid eye movement(REM)sleep latency(149.92min±67.01min vs 93.50 min±106.61 min,P<0.05),and the difference was statistically significant.Conclusion:There were abnorm al night sleep structure in patients with narcolepsy.
