摘 要:目的分析无创呼吸机辅助通气联合胸腔闭式引流治疗慢性阻塞性肺疾病(COPD)呼吸衰竭合并中-大量胸腔积液患者的临床疗效。方法选择本院2016年2月~2018年2月收治的COPD呼吸衰竭合并中-大量胸腔积液患者86例,按依据随机数字表法,分为对照组(予以无创呼吸机机械通气治疗,43例)、研究组(予以无创呼吸机辅助通气联合胸腔闭式引流治疗,43例),比较治疗前后生命体征、动脉血气指标、肺功能、住院时间。结果研究组治疗后RR、HR、PaCO2显著比对照组低,研究组治疗后PaO2、FEV1/FVC、FVC水平显著比对照组高,研究组住院时间显著比对照组短(P<0.05)。结论无创呼吸机辅助通气联合胸腔闭式引流可有效改善COPD呼吸衰竭合并中-大量胸腔积液患者的病情,维持良好的肺通气功能,缩短治疗时间,值得借鉴。Objective To analyze the clinical curative effect of non-invasive ventilator assisted ventilation combined with closed thoracic drainage in treatment of patients with COPD(chronic obstructive pulmonary disease) respiratory failure combined with moderate to massive pleural effusions. Methods 86 patients with COPD respiratory failure combined with moderate to massive pleural effusions who were admitted and treated in our hospital from February 2016 to February 2018 were selected as research objects.With the random number table method as the grouping principle,they were divided into the control group(non-invasive ventilator assisted ventilation,43 cases) and the study group(non-invasive ventilator assisted ventilation combined with closed thoracic drainage,43 cases).Vital signs, arterial blood gas index,lung function and hospital stay were compared before and after treatment. Results The RR,HR and PaCO2 in the study group were significantly lower than those in the control group after treatment.The levels of PaO2,FEV1/FVC and FVC after treatment in the study group were significantly higher than those in the control group.The hospital stay in the study group was significantly shorter than that in the control group(P<0.05). Conclusion Non-invasive ventilator assisted ventilation combined with closed thoracic drainage can effectively improve the condition of patients with COPD respiratory failure combined with moderate to massive pleural effusions,maintain good pulmonary ventilation function and shorten treatment time,which is worth learning.
