QTcd在急性颅脑损伤合并脑心综合征患者中预测室性心律失常的临床价值

摘　　要：目的探讨对于急性颅脑损伤伴脑心综合征,结合心电图检测QT离散度对室性心律失常的预测价值。方法将60例急性颅脑损伤伴脑心综合征患者根据患者是否合并室性心律失常分成两组:室性心律失常组(16例)、非室性心律失常组(44例),同时选取60例正常体检者为对照组,对3组患者均行12导联心电图检查,对各组患者QT间期离散度(QT d)、校正QT间期离散度(QT cd)指标进行观察与统计,同时分析评价心电图指标同室性心律失常的关联度。结果同非室性心律失常组与对照组相比,室性心律失常组在QT d、QT cd两项指标上均明显更高(P>0.05),其他两组无显著差异(P<0.05)。结论对急性颅脑损伤伴脑心综合征患者,QT d、QT cd会明显增加,提示临床中可将此类心电图指标作为预测室性心律失常的重要参照指标。Objective To investigate the predictive value of QT dispersion combined with electrocardiogram in detecting ventricular arrhythmias in patients with acute craniocerebral injury associated with cerebral ventricular syndrome.Methods 60 cases of acute head injury with brain-heart syndrome were divided into ventricular arrhythmia group(16 cases)and non-ventricular arrhythmia group(44 cases)according to whether the patient was combined with ventricular arrhythmia.60 cases of normal physical examination were chosen as the control group at the same time.The QT interval dispersion(QTd)and QT interval dispersion(QT cd)index of the three groups were observed and counted in 12-lead ECG.The correlation between electrocardiogram(ECG)and ventricular arrhythmias was analyzed and evaluated.Results Compared with the non-ventricular arrhythmia group and the control group,the ventricular arrhythmia group had significantly higher QT d and QTcd(P>0.05),while there was no significant difference between the other two groups(P<0.05).Conclusion For patients with acute craniocerebral injury accompanied by cerebral ventricular syndrome,QTd and QTcd can significantly increase,suggesting that such electrocardiogram indicators can be used as important reference indicators to predict ventricular arrhythmia in clinical practice.
