基底节区脑出血双管与单管引流血肿的对比观察

摘　　要：目的探究出血量在30~60 mL的基底节脑出血患者采用微创钻孔引流术单管和双管引流血肿的效果。方法回顾性分析锦州医科大学附属第一医院神经外科2016年3月至2018年9月收治的87例基底节区高血压脑出血患者,其中44例给予单管引流(单管组),43例给予双管引流(双管组),通过比较两组患者的术后再出血、带管时间、颅内感染、血肿清除率、术后第7天GCS评分及3个月mRS评分。结果单管组患者术后带管时间长于双管组(P<0.01);单管组术后颅内感染率高于双管组(P<0.05);双管组血肿清除良好率高于单管组(P<0.05);双管组术后第7天GCS评分高于单管组(P<0.05);3个月预后双管组大于单管组(P<0.05)。结论出血量在30~60 mL的基底节区脑出血患者双管引流血肿的效果优于单管。Objective To explore the effect of minimally invasive drilling and drainage by double tube and single tube on hematoma drainage in the patients with basal ganglia cerebral hemorrhage with a volume of 30~60 mL.Methods 87 cases with hypertensive cerebral hemorrhage in basal ganglia admitted to Neurosurgery Department of the First Affiliated Hospital of Jinzhou Medical University from March 2016 to September 2018 were analyzed retrospectively.44 cases were given single tube drainage(the group of single tube),and 43 patients were given double tube drainage(the group of double tube).The postoperative rebleeding,intubation time,intracranial infection,hematoma clearance rate,score of GCS on the 7th day after the surgery and score of mRS for three months of the two groups were compared.Results The postoperative intubation time in the single-tube group was longer than that in the double-tube group(P<0.01).The postoperative intracranial infection rate in the single-tube group was higher than that in the double-tube group(P<0.05).The hematoma clearance rate in the double-tube group was higher than that in the single-tube group(P<0.05).The GCS score on the 7th day after surgery of the double-tube group was higher than that of the single-tube group(P<0.05).The prognosis in the double-tube group was better than that in the single-tube group 3 months after surgery.Conclusion The effect of hematoma drainage by double tube is better than that by single tube drainage in the patients with basal ganglia hemorrhage with a blood volume of 30~60 mL.
