血清CysC用于膜性肾病病情评估的价值研究

摘　　要：目的 探讨胱抑素C(CysC)对膜性肾病(MN)患者病情评估的有应用价值。方法 选取2017年2月至2018年10月经肾脏病理学检查确诊的MN患者133例(MN组)及正常体检者53例(对照组)。检测MN患者及对照组血清尿素氮(BUN)、肌酐(Cr)、CysC、总蛋白(TP)、白蛋白(Alb)、胆固醇(CHO)、三酰甘油(TG)、磷脂酶A2受体抗体(PLA2RAb)及24 h尿蛋白。结果 MN组患者血清PLA2RAb、CysC、CHO、TG及24 h尿蛋白水平均显著高于对照组(P<0.05),TP、Alb显著低于对照组(P<0.05)。不同病理分期MN患者各实验指标无显著差异(P>0.05)。MN患者CysC与BUN、Cr、PLA2RAb及24 h尿蛋白成正相关。低蛋白尿组、中蛋白尿组与高蛋白尿组MN患者CysC水平依次升高。联合应用CysC与PLA2RAb评估PLA2RAb相关MN患者病情比单独应用具有更高预测能力。结论 血清CysC可作为MN患者病情评估的实验指标。Objective To investigate the application value of serum Cystatin C (CysC) in evaluating the severity of membranous nephropathy (MN).Methods A total of 133 patients with MN (MN group) and 53 healthy subjects (control group) were enrolled in the study.The serum levels of urea nitrogen (BUN),creatinine(Cr),CysC,total protein (TP),albumin (Alb),holesterol (CHO),triglyceride (TG),PLA2RAb and 24 hour urine protein were detected and compared betwee nthe two grousp.Results The serum levels of PLA2RAb,CysC,CHO ,TG and 24 hour urine protein in MN group were significantly higher than those in control group (P<0.05),however,the serum levels of TP and Alb in MN group were significantly lower than those in control group (P<0.05). There were no significant differences in above indexes in MN patients with different pathologic stages (P>0.05). In addition the serum levels of CysC in patients with MN were positively correlated with BUN,Cr,PLA2RAb and 24 hour urine protein,and the serum levels of CysC were successively increased with the severity of proteinuria in patients with low proteinuria,middle proteinuria,high proteinuria.The combination detection of CysC and PLA2RAb in evaluating the severity of PLA2RAb related MN was superior to that by simple index detection.Conclusion The serum CysC can be regarded as a potential biomarker in evaluating the severity of MN.
