摘 要:目的分析CYP 2C19功能缺失基因与冠状动脉粥样硬化病变的关系及性别对其的影响。方法回顾性分析2015年1月至2015年12月在广东省人民医院接受冠状动脉影像学检查的1 509例患者的临床资料,比较冠状动脉病变组与对照组CYP 2C19基因分型的差异,通过单因素及多因素回归分析明确功能缺失基因可否作为冠状动脉病变的独立危险因素。结果回顾性分析研究结果显示,冠状动脉病变组与非病变组在年龄、性别、原发性高血压(高血压)、糖尿病患病情况、吸烟、饮酒史及空腹血糖、糖化血红蛋白、三酰甘油、高密度脂蛋白胆固醇浓度等比较,差异有统计学意义(P<0.05)。男性病变组与非病变组的基因分型构成比比较,差异无统计学意义(χ^2=2.02,P=0.364);而女性病变组与非病变组的基因分型构成比比较,差异有统计学意义(χ^2=6.665,P=0.364)。对男性、女性患者基因构成作进一步分析,结果示女性患者携带CYP 2C19*3号等位基因比例低于男性,差异有统计学意义(12.5%vs. 18.1%,P<0.05)。将CYP 2C19*3号等位基因纳入多因素回归分析后结果示,*3号等位基因是冠心病预测因素(P<0.05)。结论携带CYP 2C19*3号等位基因可作为冠心病危险因素,且该风险独立于性别差异之外。Objectives To analyze the correlation between CYP 2C19 loss-of-function genes with coronary atherosclerotic heart disease and gender differences in the impact on it. Methods A retrospective analysis of clinical data of 1 509 patients who underwent coronary angiography at Guangdong Provincial People′s Hospital from January 2015 to December 2015 was performed. The differences in CYP 2C19 genotyping between coronary lesion group and control group were compared by single factor and multiple factors. Regression analysis identified whether loss-of-function genes were independent risk factors for coronary artery disease. Results Retrospective analysis showed that statistical differences were present in age,gender,hypertension,diabetes prevalence,smoking,alcohol drinking history and concentrations of fasting blood glucose,glycated hemoglobin,triglycerides,high-density lipoprotein cholesterol between coronary lesions group and control group(P<0.05). Genotype composition ratio between coronary lesion group and control group in male had no significant difference(χ~2=2.02,P=0.364);while,genotype composition ratio between coronary lesion group and control group in female had significant difference(χ~2=6.665,P=0.364). Allele ratio of CYP 2C19~*3 in female was significantly lower than that of male(12.5% vs. 18.1%,P<0.05). Taking CYP 2C19~*3 allele into multi-Logistic regression,the result indicated that CYP 2C19~*3 was an risk predictor of coronary heart disease(P<0.05).Conclusions Carrying CYP 2c19 ~*3 allele can be a risk factor of coronary heart disease,and it is independent with gender differences.
