耳内镜下鼓室成形术鼓膜穿孔修复材料的比较

摘　　要：目的探讨耳内镜下应用不同修复材料完成Ⅰ型鼓室成形术的手术技巧及疗效。方法以68例(68耳)全耳内镜下进行Ⅰ型鼓室成形术的慢性化脓性中耳炎患者为研究对象,术中分别采用耳屏软骨膜(12例)、耳屏单层全厚软骨(26例)及耳屏软骨膜半厚软骨复合体(30例)为鼓膜修复材料,分析比较3种修复材料的取材难易程度、术后鼓膜再穿孔率及听力改善疗效。结果12例耳屏软骨膜修复的患者中有2例术后鼓膜再穿孔(16.67%),26例耳屏全厚软骨修复者均无术后鼓膜再穿孔,但鼓膜标志显示欠佳;30例耳屏软骨膜半厚软骨复合体修复者中有1例鼓膜再穿孔(3.33%),鼓膜形态显示良好;术后听力提高程度各组无统计学差异(P>0.05)。结论本组耳内镜下Ⅰ型鼓室成形术鼓膜修复材料中耳屏软骨膜半厚软骨复合体取材便利,操作简易,术后穿孔率低,鼓膜形态良好。Objective To compare the efficacy of three kinds of repair materials for type I tympanoplasty under oto-endoscopy.Methods A total of 68 patients(68 ears)with chronic suppurative otitis media underwent tympanoplasty under total ear endoscope were selected as the study subjects.The tragus perichondrium(12 cases),tragus monolayer full thickness cartilage(26 cases)and tragus perichondrium semi-thick cartilage complex(30 cases)were used as tympanic membrane repair materials during the operation.The degree of difficulty in obtaining materials,the rate of re-perforation of tympanic membrane and the effect of hearing improvement after operation were analyzed and compared.Results Twelve patients with tragus perichondrium had postoperative perforation perforation(16.67%),26 patients with tragus full-thickness cartilage repair had no tympanic membrane perforation after operation,and 1 patient with tragus perichondrium semi-thick cartilage complex repair had tympanic membrane perforation(3.33%).There was no statistical difference in the degree of hearing improvement among the 3 groups(P>0.05).Conclusion Among the materials for tympanoplasty repair under total ear endoscopy,the tragus perichondrium semithick cartilage complex is easy to obtain and operate,and the perforation rate is low,and the tympanic membrane shape is good.
