摘  要:目的观察益气活血化痰法对急性呼吸衰竭模型大鼠肺功能、细胞生长因子及肺表面活性蛋白B(SP-B)表达的影响。方法将60只雄性SD大鼠分为对照组、模型组、中药组及阳性组,每组15只,除对照组外各组均采用毒素脂多糖建立急性呼吸衰竭大鼠模型。对照组和模型组给予3 mL/kg的生理盐水灌胃,中药组给予3 mL/kg的益气活血化痰方灌胃,阳性组给予5 mg/kg地塞米松灌胃。干预12 d后评价肺功能,比较肺组织病理学改变,血清血管内皮生长因子(VEGF)和碱性成纤维细胞生长因子(bFGF)水平,肺组织表面活性蛋白B(SP-B)表达。结果与对照组比较,模型组FEV0.3/FVC值显著降低,呼气阻力显著升高(P <0.05)。与模型组比较,中药组和阳性组FEV0.3/FVC值显著高于模型组,呼气阻力显著低于模型组(P <0.05)。对照组肺组织未见明显异常;模型组结构被破坏,明显损伤,间质水肿,大量炎性细胞浸润,组织间充满渗出液;中药组和阳性组结构相对完整,水肿较轻,渗出较少,少量炎性细胞浸润。与对照组比较,模型组VEGF和bFGF水平显著升高;与模型组比较,中药组和阳性组VEGF和bFGF水平显著降低(P <0.05)。与对照组比较,模型组肺湿重/干重(W/D)比值显著升高,SP-B表达水平显著降低;与模型组比较,中药组和阳性组W/D比值显著降低,SP-B表达水平显著升高(P <0.05)。结论益气活血化痰方可改善急性呼吸衰竭模型大鼠肺功能,可能与降低细胞生长因子及肺SP-B蛋白表达有关。Objective:To study the effects of Yiqi Huoxue Huatan Method on lung function,cell growth factor and lung SP-B protein expression in rats with acute respiratory failure. Methods:60 male SD rats were divided into the control group,the model group,TCM group and the positive group,15 rats in each. The acute respiratory failure rat models were established by toxin lipopolysaccharide except the control group. The control group and the model group were given 3 mL/kg of saline,the TCM group 3 mL/kg of invigorating Yiqi Huoxue Huatan Decoction,and the positive group 5 mg/kg of dexamethasone. The pulmonary function was evaluated after 12 days′ intervention. The pathological changes of lung tissue,the levels of serum VEGF and bFGF and the expression of SP-B in lung tissue were compared. Results:Compared with the control group,FEV0.3/FVC value of the model group decreased significantly and the expiratory resistance increased significantly(P < 0.05). Compared with the model group,that of the TCM group and the positive group was significantly higher than that of the model group,and the expiratory resistance was significantly lower than that of the model group(P < 0.05). There was no obvious abnormality in the control group;the structure of the model group was damaged incompletely,with obvious damage,interstitial edema,a large number of inflammatory cells infiltration and interstitial exudates filled;the structure of the TCM group and the positive group was relatively complete,with slight edema and less infiltration of inflammatory cells. Compared with the control group,the levels of VEGF and b FGF in the model group were significantly higher,and those in the TCM group and the positive group were significantly lower than the model group(P<0.05).Compared with the control group,the W/D ratio of the model group was significantly higher and the expression of SP-B was significantly lower.
