益气活血法配合无创呼吸机对特发性肺纤维化患者肺功能影响的临床观察

摘　　要：目的观察益气活血法配合无创呼吸机对特发性肺纤维化(IPF)患者肺功能指标的影响并探讨其机制。方法纳入IPF患者50例,采用随机数字表法分为观察组和对照组各25例,对照组采用常规西药治疗,根据病情使用无创呼吸机;观察组在对照组治疗基础上均使用无创呼吸机,同时加用益气活血中药治疗,疗程14 d,随访1个月,比较两组患者治疗效果、动脉血气分析和肺功能指标的情况,并记录两组患者的不良反应。结果治疗后观察组总有效率高于对照组,差异有统计学意义(P <0.05);治疗后,两组中医证候积分比较,差异有显著统计学意义(P <0.01);治疗后,动脉血氧分压(PaO2)两组比较,差异有统计学意义(P <0.05),观察组优于对照组,PaCO2两组比较,差异无统计学意义(P> 0.05);治疗后观察组FEV1%预计值、FEV1/FVC测值均高于对照组,差异有统计学意义(P <0.05)。结论益气活血法配合无创呼吸机能改善IPF患者的动脉血气、肺功能指标,可以提高患者治疗效果,且无不良反应发生。Objective:To observe the effect of Yiqi Huoxue Method combined with noninvasive ventilator on pulmonary function indexes of patients with IPF. Methods:50 patients were randomly divided into the observation group and the control group,25 cases in each. The control group was treated with conventional Western medicine and non-invasive ventilator according to the condition of the patients;the observation group was treated with noninvasive ventilator on that basis,and added with Yiqi Huoxue Chinese medicine. The therapeutic effect,ABG analysis and pulmonary function indexes of the two groups were compared,and the adverse reactions of the two groups were recorded. Results:After treatment,there was significant difference in the total effective rate(P < 0.05),TCM syndrome integral(P < 0.01)and PaO2(P < 0.05);the observation group was better than the control group. There was no significant difference in PaCO2 between the two groups(P > 0.05). After treatment,the FEV1% predicted value and FEV1/FVC measured value of the observation group were higher than those of the control group;the difference was statistically significant(P < 0.05). Conclusion:Yiqi Huoxue Method combined with noninvasive ventilator can improve ABG indexes,pulmonary function and the therapeutic effect of IPF patients,without adverse reactions.
