钢板螺钉内固定手术治疗老年桡骨远端不稳定骨折患者的临床效果

摘　　要：目的:探讨钢板螺钉内固定手术治疗老年桡骨远端不稳定骨折患者的临床效果。方法:选取老年桡骨远端不稳定骨折患者130例,按照随机数字表法将其分为观察组与对照组,每组65例。对照组予保守治疗,观察组予Lane钢板螺钉内固定手术治疗,比较两组患者住院时间和骨折愈合时间,腕关节功能自行评估量表(PRWE)评分,治疗前后腕关节活动度及并发症发生率。结果:观察组患者住院时间和骨折愈合时间均短于对照组,PRWE评分高于对照组,差异均有统计学意义(P<0.05);治疗3个月后,两组患者患侧握力、背伸活动度、掌屈活动度、尺偏活动度、桡偏活动度、旋前及旋后活动度均大于治疗前,且观察组大于对照组,差异有统计学意义(P<0.05);观察组患者并发症发生率为4.69%,低于对照组的14.06%,差异有统计学意义(P<0.05)。结论:钢板螺钉内固定手术治疗老年桡骨远端不稳定骨折患者临床效果较好,可改善患者腕关节功能,并降低并发症发生率,预后良好。Objective: To investigate clinical effects of steel plate screw internal fixation on elderly patients with unstable distal radius fractures. Methods: 130 elderly patients with unstable distal radius fractures were selected and divided into observation group and control group according to the random number table method, with 65 cases in each group. The control group was treated conservatively, while the observation group was treated with Lane plate screw internal fixation. The hospitalization time and fracture healing time, the patient rated wrist evaluation(PRWE) score, wrist range of motion before and after the treatment, and the complication rate were compared between the two groups. Results: The hospitalization time and fracture healing time of the observation group were shorter than those of the control group, the PRWE score was higher than that of the control group, and the differences were statistically significant(P<0.05). 3 months after the treatment, the affected side’s grip strength as well as dorsiflexion, palm flexion, ulnar deviation, radial deviation, pronation and supination ranges of motion of the two groups were greater than those before the treatment, those of the observation group were greater than those of the control Group, and the differences were statistically significant(P<0.05). The incidence of complications in the observation group was 4.69%, which was lower than 14.06% in the control group, and the difference was statistically significant(P<0.05). Conclusions: For the elderly patients with unstable distal radius fractures, the steel plate screw internal fixation is effective. Moreover, it can improve the wrist function and reduce the incidence of complications with good prognosis. Therefore, it is worthy of clinical recommendation.
