揿针促进加速康复外科老年结直肠癌术后胃肠功能恢复的临床研究

摘　　要：目的观察揿针疗法对加速康复外科老年结直肠癌术后胃肠功能恢复的疗效。方法选取全麻老年结直肠癌根治术后63例为研究对象,随机分为观察组(n=32)和对照组(n=31),2组均按照ERAS方案进行治疗护理,观察组同时予揿针辅助治疗,观察并比较2组在术后0~6 h、6~24 h、24~48 h和48~72 h不同时间段的腹胀及胃肠反应(GIR)分级,肛门首次排气和排便时间,出院患者满意度等。结果 2组腹胀在24~48h和48~72h时间段比较,差异有统计学意义(P<0.05);胃肠反应(GIR)分级在0~6 h和24~48 h时间段比较,有统计学意义(P<0.05);2组肛门首次排气和排便时间差异有统计学意义(P<0.05)。结论揿针辅助治疗能在术后24~48 h和48~72h时间段有效缓解腹胀,在0~6 h和24~48 h时间段能够缓解恶心呕吐,同时也促进术后肛门排气和排便,促进老年结直肠癌患者术后胃肠功能恢复。Objective To observe the curative effect of thumbtack needle therapy in promoting gastrointestinal function recovery of the elderly with colorectal cancer in enhanced recovery after surgery(ERAS).Methods 63 elderly after radical resection for colorectal cancer under general anesthesia were enrolled and were randomly divided into 2 groups:observation group(n=32) and control group(n=31);ERAS scheme was performed in the treatment and nursing of the elderly in both group while thumbtack needle therapy was added to the elderly in observation group;abdominal distension,gastrointestinal reaction(GIR)grading,the times of first anal exhaust and defecation in the time quantum of post-operative 0 to 6 hours,6 to 24 hours,24 to48 hours and 48 to 72 hours,and the satisfaction of the elderly at discharge were observed and a comparative study was made between the 2 groups.Results There existed a statistical difference in abdominal distension in the time quantum of post-operative24 to 48 hours and 48 to 72 hours(P<0.05),a statistical difference in GIR grading in the time quantum of 0 to 6 hours and 24 to 48 hours(P<0.05),and a statistical difference in the times of first anal exhaust and defecation(P<0.05) between the 2 groups.Conclusions Thumbtack needle therapy can effectively relieve the abdominal distension in the time quantum of post-operative 24 to 48 hours and 48 to 72 hours,relieve the symptom of nausea and vomiting in the time quantum of post-operative 0 to 6 hours and 24 to 48 hours,it can promote post-operative anal exhaust and defecation and the gastrointestinal function recover of the elderly with colorectal cancer after surgery.
