阿替洛尔治疗老年心律失常的临床观察

摘　　要：目的探讨阿替洛尔在老年心律失常中的临床疗效,为治疗老年心律失常提供方法。方法选择2017年7月-2019年1月我院收治的老年心律失常患者158例,按随机数字表法分为对照组(n=79)和观察组(n=79)。对照组给予胺碘酮治疗,观察组在对照组基础上联合阿替洛尔治疗。连续治疗2周后,评估并比较2组治疗效果、心率变异率和生活质量,并于治疗结束后进行随访12个月,观察比较2组治疗预后。结果观察组治疗后SDNN、SDANN、RMSSD、LF及HF水平均显著高于对照组(P<0.05),生活质量中生理、心理、独立性、环境及社会关系领域评分均显著高于对照组(P<0.05)。随访期间,观察组治疗后再发心律失常、心肌梗死、心力衰竭和死亡率与对照组差异均无统计学意义(P>0.05)。结论阿替洛尔用于治疗老年心律失常有助于降低患者心率变异率,提高生活质量,改善预后,具有一定的临床应用价值。Objective To explore the curative effect of atenolol on arrhythmia in the elderly.Methods 158 elderly with arrhythmia hospitalized during the period from July,2017 to Jan.,2019 were selected and randomly divided into 2 groups:control group and observation group,79 in each;amiodarone was applied to the elderly in both groups while combination with atenolol was added to the elderly in observation group;the courses of the 2 groups lasted for 2 weeks;the curative effect,the heart rate variability and quality of life after treatment were evaluated and a comparative study was made between the 2 group;follow-up covered the 12 months after treatment and the prognosis of the elderly in the 2 groups were observed and compared.Results The levels of SDNN,SDANN,RMSSD,LF,HF and the scores of physical,psychological,independent,environmental and social relationship in quality of life in observation group were significantly higher than those in control group(P<0.05);no statistical difference was found between the 2 groups in recurrence of arrhythmia,myocardial infarction,heart failure and mortality during the follow-up(P>0.05).Conclusions Atenolol is effective in reducing the heart rate variability,promoting the quality of life and improving the prognosis of the elderly with arrhythmia,worthy of certain clinical application.
