雌三醇乳膏联合诺氟沙星胶囊治疗女性老年泌尿生殖道感染的临床观察

摘　　要：目的探讨雌三醇乳膏联合诺氟沙星胶囊对治疗女性老年泌尿生殖道感染的效果,为治疗该病提供方法。方法选取2016年1月-2018年1月我院就诊的老年女性泌尿生殖道感染患者92例,采用随机数字表法分为对照组和观察组,每组46例。对照组采用诺氟沙星胶囊治疗,观察组采用诺氟沙星胶囊+雌三醇乳膏治疗。观察并比较2组疗效、不良反应、治疗前后阴道炎症状及健康评分、pH值、雌二醇(E2)、子宫内膜厚度、血清免疫球蛋白水平[免疫球蛋白A (IgA)、免疫球蛋白G (IgG)、免疫球蛋白M (IgM)]等情况,并于治疗后3个月和6个月进行随访,评估并比较2组复发率。结果 (1)观察组治疗总有效率明显高于对照组(93.48%vs 73.91%,P<0.05);(2)治疗后观察组阴道健康状况评分明显高于对照组(P<0.05),而阴道炎症状评分显著低于对照组(P<0.05);(3)治疗前、治疗后2组E2水平和子宫内膜厚度差异均无统计学意义(P>0.05),治疗后观察组pH值明显低于对照组(P<0.05);(4)治疗后观察组血清IgA、IgG和IgM水平均明显高于对照组(P<0.05);(5)观察组不良反应发生率与对照组差异无统计学意义(P>0.05);(6) 2组均无脱落病例。治疗后3个月和6个月2组复发率差异均无统计学意义(P>0.05)。结论雌三醇乳膏联合诺氟沙星胶囊治疗老年泌尿生殖道感染女性疗效显著,可上调血清免疫球蛋白,降低阴道pH值,且安全性高,具有一定的临床应用价值。Objective To explore the clinical effect of estriol cream combined with norfloxacin capsule in the treatment of genitourinary tract infection in elderly females.Methods 92 elderly females with genitourinary tract infection during the period from January,2016 to January,2018 were divided into 2 groups:control group and observation group,46 cases in each;norfloxacin capsule was provided to cases in both groups while the combination with estriol cream was added to cases in observation group;observation was made to the effective rate,incidence of adverse reactions,the scores of vaginal inflammation symptoms and vaginal health,pH value,estradiol(E2) level,endometrial thickness,serum immunoglobulin levels(IgA,IgG,IgM) of all the cases after treatment and a comparative study was made between the 2 groups;recurrence was followed up 3 months and 6 months after treatment.Results(1) The total effective rate in observation group was higher than in control group(93.48% vs.73.91%,P<0.05);(2) After treatment,the vaginal health scores were higher and the scores of vaginitis symptoms were lower in observation group than those in control group(P<0.05);(3) There was no statistical difference in E2 level and endometrial thickness between the 2 groups before and after treatment(P>0.05),and the pH values of the cases in observation group were lower than those in control group(P<0.05);(4) The levels of serum IgA,IgG and IgM of the cases in observation group were higher than those in control group after treatment(P<0.05);(5) There was no statistical difference in the incidence of adverse reactions between the 2 groups(P>0.05);(6) No case in both groups dropped out;no statistical difference was found in recurrence between the 2 groups 3 months and 6 months after treatment(P>0.05).Conclusions Estradiol cream combined with norfloxacin capsules is effective and safe in the treatment of genitourinary tract infection in elderly females,it can up-regulate the level of serum immunoglobulin and reduce vaginal pH value;it is worthy of certain cli
