摘  要:目的通过探讨子痫前期患者外周血内皮细胞微粒(endothelial microparticles,EMPs)与CD 8^+ CD 25^+ FoxP 3^+调节T细胞(regulatory T cell,Treg)之间关系,阐明EMPs对Treg功能影响及其在子痫前期发病过程中的机制。方法选取2017年6月至2018年5月在上海市嘉定区妇幼保健院行产检的子痫前期孕妇60例为研究组,其中38例非重度子痫前期患者为A组,22例重度子痫前期患者为B组;同时随机选取60例健康孕妇为对照组。收集所有孕妇外周血,应用流式细胞仪检测外周血EMPs、CD 8^+ CD 25^+ FoxP 3^+ Treg、FoxP 3 mRNA表达水平及转化生长因子-β(transforming growth factor-β,TGF-β)、白介素(Interleukin,IL)-10、IL-6、IL-17血清浓度。比较各组之间以上指标差异并研究其相关性。结果与对照组比较,A、B组EMPs水平显著升高,且B组高于A组,差异有统计学意义(P<0.05)。与对照组比较,A、B组CD 8^+ CD 25^+ FoxP 3^+ Treg比例及FoxP 3 mRNA表达水平显著降低,且B组较A组更低,差异有统计学意义(P<0.05)。与对照组比较,A、B组TGF-β、IL-6、IL-17的血清浓度均升高,其中IL-6、IL-17在B组中水平更高,差异有统计学意义(P<0.05)。TGF-β在A组与B组间差异无统计学意义(P>0.05)。3组患者IL-10水平比较,差异无统计学意义(P>0.05)。相关性分析结果显示,B组EMPs水平与CD 8^+ CD 25^+ FoxP 3^+ Treg、FoxP 3 mRNA表达水平呈负相关(r=-0.379、-0.427,P=0.001、0.004),与TGF-β、IL-6、IL-17呈正相关(r=0.376、0.427、0.257,P=0.024、0.005、0.037),与IL-10不相关(r=0.173,P=0.217)。结论 EMPs在子痫前期患者血浆表达水平上升,在子痫前期发病机制中起到一定的作用,为临床诊断及治疗提供新的思路。Objective To investigate the relationship between endothelial microparticles(EMPs) and CD 8^+CD 25^+FoxP 3^+ regulatory T cells(Treg) in patients with preeclampsia, and to elucidate the effects of EMPs on T regulatory cell function and its mechanism in the pathogenesis of preeclampsia. Methods 60 pre-eclampsia pregnant women who underwent birth examination in Maternal and Child Health Hospital of Jiading District from June 2017 to May 2018 were enrolled as study group, 38 of whom were mild pre-eclampsia(group A) and 22 were severe pre-eclampsia(Group B), 60 healthy pregnant women were randomly selected as the control group. Peripheral blood was collected from all subjects, and peripheral blood EMPs, CD 8^+CD 25^+FoxP 3^+ Treg, FoxP 3 mRNA expression levels and transforming growth factor-β(TGF-β) were detected by flow cytometry. Serum concentrations of interleukin-10(IL-10), IL-6, IL-17 were detected. The differences between the above indicators were compared between groups and their correlations were studied. Results Compared with the control group, the levels of EMPs in group A and group B increased significantly and group B was higher than that in group A, and the difference were statistically significant(P<0.05).Compared with the control group, the ratio of CD 8^+CD 25^+FoxP 3^+ Treg and FoxP 3 mRNA expression in group A and group B were significantly lower, and group B was lower than group A, the differences were statistically significant(P<0.05). Compared with the control group, the serum concentrations of TGF-β, IL-6 and IL-17 were significantly increased in group A and group B, among which IL-6 and IL-17 were higher in group B, and the difference was statistically significant(P<0.05). There was no significant difference in TGF-β between group A and group B(P>0.05). There was no significant difference in IL-10 levels between the three groups(P>0.05). Correlation analysis showed that the level of EMPs in group B was negatively correlated with CD 8^+CD 25^+FoxP 3^+ Treg and FoxP 3 mRNA expression levels(r
