摘  要:目的观察针刺太冲穴及其不同配穴对人脑功能成像的影响,探讨穴位配伍效应的相关机制。方法40例健康受试者随机分为右太冲组、双太冲组、右太冲配右内关组、右太冲配右光明组4组,每组10例。在手法捻针刺激相应穴位基础上,两组均按照组块设计方案刺激与静留针交替进行,同时针刺过程中fMRI扫描两组静息态脑功能图像,所获得数据通过Matlab软件SPM12模块分析效应脑区。结果右太冲组针刺后主要为对侧脑区耗氧量增高,激活左侧腹内前额叶(内侧前额叶/前扣带回)、左侧颞中回、右侧颞下回、左侧额中回、左侧脑干。双太冲组主要激活了左侧小脑、右侧丘脑、后扣带回、左侧枕叶梭状回、左侧楔叶。右太冲配右光明组主要激活了右侧小脑、右侧枕叶梭状回、右侧舌回、右侧额中回。右太冲配右内关组激活了右侧枕中回、右侧楔叶。右太冲组及双太冲组未见到脑功能活动显著抑制的区域;右太冲配右光明组脑功能活动被抑制的主要区域为双侧颞下回;右太冲配右内关组脑功能活动被抑制的脑区在左侧小脑、右侧缘上回、左侧额下回、左侧枕叶梭状回。结论针刺太冲单穴及其不同配穴的脑功能效应有显著差异,太冲配穴会广泛影响与视觉有关的枕叶,另外小脑可能参与了针刺信号在大脑中的整合作用,并且与配穴的规律有关。Objective To observe the effects of acupuncture at Taichong( LR3) and adjunct acupoints on brain functional imaging in healthy people,and to explore the related mechanism of acupoints compatibility. Methods A total of 40 healthy subjects were randomly divided into right Taichong( LR3) group,bilateral Taichong( LR3)group,right Taichong( LR3) plus right Neiguan( PC6) group,right Taichong( LR3) plus right Guangming( GB37)group,with 10 subjects in each group. On the basis of twisting needle stimulation,both groups were alternately stimulated and retained needles according to block design. At the same time,functional magnetic resonance imaging( fMRI) scanned the resting brain functional imaging during the needling process. The data obtained were used to analyze affected brain area by SPM12 module of MATLAB software. Results Acupuncture at right Taichong( LR3) mainly caused the increase of oxygen consumption in the contralateral brain area,and activated the left ventromedial prefrontal lobe( medial prefrontal lobe/anterior cingulate gyrus),left middle temporal gyrus( MTG),right inferior temporal gyrus( ITG),left middle frontal gyrus( MFG) and left brainstem. In the bilateral Taichong( LR3)group,the main activated areas were left cerebellum,right thalamus,posterior cingulate gyrus,left fusiform gyrus of occipital lobe( OFuG) and left cuneiform lobe. In right Taichong( LR3) plus right Guangming( GB37) group,the main activated areas were right cerebellum,right OFuG,right lingual gyrus( li G) and right MFG. In right Taichong( LR3) plus right Neiguan( PC6) group,the areas of brain activation were the right middle occipital gyrus and right cuneus. No significant inhibition of brain function was observed in the right Taichong( LR3) group and the bilateral Taichong( LR3) group;the main inhibited area of brain function in the right Taichong( LR3) plus right Guangming( GB37) group was bilateral ITG;The inhibited area of brain function in the Right Taichong( LR3) plus right Neiguan( PC6) group was the left cerebellum,the right su
