摘　　要：目的评价产房急诊5分钟剖宫产模拟急救演练的效果。方法2017年1至6月西安市第四医院对产房医护人员进行产房急诊5分钟剖宫产模拟急救演练的全员培训共7次,参加人员79名,选取其中51名助产士进行问卷调查,评价其有效性,比较实施演练前后临床应急操作能力及理论成绩。结果开展产房急诊5分钟剖宫产模拟急救演练后,51名助产士理论考核及应急操作考核成绩(病情判断能力、解决问题能力、整体抢救流程、护患沟通能力、配合协调能力)均高于演练前,差异均有统计学意义(t值分别为14.448、18.321、17.652、17.962、17.017、17.358,均P<0.05)。51名助产士完成产房急诊5分钟剖宫产模拟急救演练后,应急操作能力、急救仪器的使用能力、理论联系实际能力的可提高/有效的比例均高于不可提高/无效的比例,其差异均有统计学意义(χ~2值分别为29.315、31.235、33.342,均P<0.05);应急处理能力和团队协作力的可提高/有效的比例也均高于不可提高/无效的比例,其差异均有统计学意义(Fisher确切概率法,P值均为0.000)。结论模拟演练对提升产房急诊5分钟剖宫产的质量有着重要的意义,且能提高医护人员评估病情能力、抢救应急能力及增强团队凝聚力,值得进一步推广应用。Objective To evaluate the effect of simulated first aid training of 5-minute cesarean section in the delivery room.Methods From January to June 2017,the Fourth Hospital of Xi’an conducted 7 training sessions to the medical staff in the delivery room in simulated 5-minute cesarean section in the delivery room.79 participants were selected and 51 midwives from them were surveyed by questionnaires to evaluate the effectiveness and compare their clinical emergency operation ability and theoretical performance before and after the training.Results After the training,the scores of 51 midwives in theory assessment and emergency operation assessment(abilities in disease judgment,problem solving,overall rescue process following,nurse-patient communication,and coordination)were higher than those before the drilling,and the differences were statistically significant(t=14.448,18.321,17.652,17.962,17.017,17.358,respectively,P<0.05 for all).Also,the ratios of improvement/effectiveness in the abilities to perform emergency procedure,use emergency equipment,and combine theory with practice to improve/effectively weres higher than those of non-improvement/ineffectiveness,and the difference was statistically significant(χ~2=29.315,31.235 and 33.342,respectively,P<0.05).The ratios of improvement/effectiveness of emergency response ability and team cooperation ability were also higher than those of non-improvement/inefficiency,and the difference was statistically significant in Fisher’s exact test(P=0.000).Conclusion Simulated exercise is of great significance to improve the quality of emergency 5-minute cesarean section in delivery room.It can also improve the ability of medical staff to assess the disease condition,rescue and emergency response,and enhance team cohesion.It is worthy of further promotion and application.
