摘 要:目的探讨Endocan因子对冠状动脉慢性完全闭塞(chronic total occlusion,CTO)患者侧枝循环生成的影响。方法44例CTO患者根据冠状动脉侧支循环情况分为冠状动脉侧枝循环劣势组24例和优势组20例,比较2组患者血清Endocan因子水平。雄性10周龄C57BL/6J小鼠20只,分为对照组和Endocan组各10只,结扎2组小鼠左后肢腹股沟韧带以下股动脉后,对照组小鼠左后肢肌内注射生理盐水1mL/次,1次/3d;Endocan组小鼠左后肢肌内注射Endocan因子1μg/kg,1次/3d;14d后应用激光多普勒血流成像仪检测2组小鼠缺血侧与非缺血侧后肢血流灌注值。结果冠状动脉侧枝循环优势组患者血清Endocan表达水平[(418.69±81.10)μg/L]明显高于劣势组[(270.23±42.19)μg/L](P<0.05);14d后2组小鼠非缺血侧后肢血流灌注值均高于缺血侧(P<0.05);Endocan组缺血侧后肢血流灌注值(625.80±121.25)和缺血侧/非缺血侧后肢血流灌注比值(0.64±0.10)均高于对照组(389.40±64.15、0.38±0.03)(P<0.05),非缺血侧后肢血流灌注值(967.90±109.50)与对照组(1018.00±139.24)比较差异无统计学意义(P>0.05)。结论Endocan因子具有促进血管新生作用,有望成为CTO患者改善侧枝循环的新治疗靶点。Objective To investigate the effect of endocan on coronary collateral circulation in patients with chronic total occlusion(CTO).Methods According to the coronary collateral circulation,44CTO patients were divided into inferior group(n=24)and superior group(n=20).The serum levels of endocan were detected in two groups.Twenty male C57BL/6Jmice aged 10weeks old were divided into control group and endocan group,with 10mice in each group.The femoral arteries under inguinal ligament of left hindlimb was ligated.Control group was injected with 1mL normal saline intramuscularly in left hindlimb,once per three days.Endocan group was injected with 1μg/kg endocan factor intramuscularly in left hindlimb,once per three days.After 14days,the perfusion value of blood flow was detected in ischemic and non-ischemic hindlimb by laser Doppler flow imaging.Results The expression level of endocan was significantly higher in superior group((418.69±81.10)μg/L)than that in inferior group((270.23±42.19)μg/L)(P<0.05).The perfusion value of blood flow was higher in non-ischemic hindlimb than that in ischemic hindlimb in both groups(P<0.05),was higher in ischemic hindlimb in endocan group(625.80±121.25)than that in control group(389.40±64.15)(P<0.05),and showed no significant difference in non-ischemic hindlimb between two groups(967.90±109.50 vs.1018.00±139.24)(P>0.05).The perfusion value ratio of ischemic to non-ischemic hindlimb blood flow was higher in endocan group(0.64±0.10)than that in control group(0.38±0.03)(P<0.05).Conclusion Endocan may be a new therapeutic target for improving collateral circulation in CTO patients by promoting angiogenesis.
