塞来昔布对骨关节炎的临床疗效及对患者精子质量的影响

摘　　要：目的验证塞来昔布对骨关节炎的临床疗效,同时观察其对受试者精子质量的影响。方法骨关节炎男性患者70例随机分为治疗组和对照组各35例。治疗组口服塞来昔布200mg,每日1次。对照组使用超声治疗仪,每次治疗20min,每日2次。2组患者采用相同运动处方和运动疗法,疗程均为8周,1周随访1次。在治疗前和治疗4周、8周后分别进行疼痛视觉模拟量表(VAS)、日本骨科协会评估治疗(JOA)和美国西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分,并进行精子质量检测。结果2组VAS、JOA和WOMAC评分在治疗4周、8周后均较治疗前显著改善(均P<0.01),其中治疗组患者比对照组改善更加显著(P<0.01)。治疗组总有效率94%,对照组总有效率71%,组间有显著差异(P<0.05)。治疗组精子DNA完整率在治疗4周、8周后分别为(84.5±4.3)%、(82.4±4.2)%,正常精子比例分别为(5.82±0.60)%、(5.83±0.67)%,2组数值均在正常范围内,且与治疗前比较均无显著差异(P>0.05)。结论塞来昔布对骨关节炎治疗有效且未对患者精子质量造成显著影响。AIM To verify the clinical efficacy of celecoxib on osteoarthritis,and to observe its effect on sperm quality.METHODS A total of 70 male patients who were diagnosed with osteoarthritis were randomly divided into treatment group and control group.The treatment group was given celecoxib capsule 200 mg,once a day.The control group was treated with ultrasound therapy instrument for 20 minutes,twice a day.The course of treatment was 8 weeks,and both groups were treated with the same exercise prescription and exercise therapy,followed up once a week.Before treatment and 4 or 8 weeks after the treatment,the pain Visual Analogue Scale(VAS),Japanese Orthopaedic Association(JOA)Score,and Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)were estimated,and sperm quality was tested.RESULTS The scores of VAS,JOA and WOMAC of the two groups were significantly improved after 4 weeks and 8 weeks of treatment(all P<0.01)compared with those before treatment,and the improvement of the treatment group was more significant than that of the control group(P<0.01).The total effective rate of the treatment group was 94%and the control group was 71%,with significant difference between the two groups(P<0.05).After 4 and 8 weeks of treatment,the sperm DNA integrity rate of the treatment group was(84.5±4.3)%and(82.4±4.2)%respectively,and the normal sperm morphology rate was(5.82±0.60)%and(5.83±0.67)%respectively.The values were within the normal range in the two groups,and there was no significant difference before and after treatment(P>0.05).CONCLUSION Celecoxib is effective in the treatment of osteoarthritis,and has no significant effect on sperm quality.
