两种内镜防护法在康派特医用胶联合内镜治疗胃溃疡出血中的应用价值分析

摘　　要：目的比较碘化油、自制透明胶套两种内镜防护法在康派特医用胶联合内镜治疗胃溃疡出血中的应用价值。方法240例因胃溃疡出血实施内镜止血的患者,随机分为碘化油组和自制透明胶套组,每组120例。两组患者均采用内镜下喷洒康派特医用胶治疗,碘化油组采用碘化油涂抹内镜镜头防护,自制透明胶套组采用自制透明胶套固定镜头防护。比较两组内镜保护情况、胃镜止血情况及止血效果。结果自制透明胶套组内镜粘胶率0、内镜返厂维修率4.17%低于碘化油组的15.00%、20.00%,内镜清洗时间(17.76±5.67)min短于碘化油组的(30.31±7.43)min,差异均具有统计学意义(P<0.05)。两组患者呕血消失时间、黑便消失时间、24 h快速止血率以及再出血率比较差异无统计学意义(P>0.05)。两组患者止血总有效率比较差异无统计学意义(P>0.05)。结论碘化油、自制透明胶套两种内镜防护方法在康派特医用胶联合内镜治疗胃溃疡出血患者中均有一定的应用价值,但自制透明胶套内镜防护方法能更加有效地降低粘胶、堵塞镜头风险,并能有效减少镜头清洗时间,值得临床推广。Objective To compare the value of two kinds of endoscopic protection methods of iodized oil and self-made transparent rubber sleeve in the treatment of gastric ulcer bleeding with Compont medical glue combined with endoscopy.Methods A total of 240 cases of endoscopic hemostasis due to gastric ulcer bleeding was randomly divided into iodized oil group and self-made transparent rubber sleeve group,with 120 cases in each group.Both groups was treated by spraying Compont medical glue under the endoscope,and the iodized oil group was protected by applying iodized oil to the endoscope lens.The self-made transparent rubber sleeve group was protected by fixing the lens with self-made transparent rubber sleeve.The endoscopic protection,hemostasis of gastroscope and hemostasis effect were compared between the two groups.Results The endoscopic glue rate and endoscope return factory maintenance rate were 0 and 4.17%in self-made transparent rubber sleeve group,which was lower than 15.00%and 20.00%in iodized oil group,and endoscope cleaning time(17.76±5.67)min was shorter than(30.31±7.43)min in iodized oil group.Their difference was statistically significant(P<0.05).There was no statistically significant difference in disappearance time of hematemesis and black stool,24 h rapid hemostasis rate and rebleeding rate between the two groups(P>0.05).There was no statistically significant difference in total effective of hemostasis between the two groups(P>0.05).Conclusion The two kinds of endoscopic protection methods of iodized oil and self-made transparent rubber sleeve show certain application value in the treatment of gastric ulcer bleeding patients with Compont medical glue and endoscopy,but the selfmade transparent rubber sleeve endoscopic protection method can more effectively reduce the risk of glue and blockage of lens,and can effectively reduce lens cleaning time,which is worthy of clinical promotion.
