妊娠期代谢综合征孕妇内脂素、脂联素、血管细胞粘附分子-1水平与妊娠不良结局的相关性

摘　　要：目的探究妊娠期代谢综合征孕妇内脂素、脂联素(APN)及血管细胞粘附分子-1(VCAM-1)表达水平与高危妊娠母儿结局的相关性。方法回顾性选取2016年6月~2018年6月唐山市妇幼保健院产科收治住院的60例合并妊娠期代谢综合征孕妇作为观察组以及60例健康孕妇作为对照组。检测两组母体血清、胎盘组织中的内脂素、APN、VCAM-1表达水平及相关代谢参数,对比两组妊娠结局,分析各指标与不良妊娠结局的相关性。结果观察组(妊娠期代谢综合征妊娠孕妇)血清及胎盘组织中的内脂素、VCAM-1表达水平显著的高于对照组(P<0.05),APN显著低于对照组(P<0.05);观察组血清LDL-C、TC、TG、FPG、HOMA-IR显著的高于对照组(P<0.05),HDL-C显著的低于对照(P<0.05);观察组的巨大儿、早产儿发生率显著高于对照组(P<0.05)。内脂素、VCAM-1与妊娠期代谢综合征孕妇是否发生不良妊娠结局显著正相关,APN呈显著负相关。结论妊娠期代谢综合征孕妇内脂素、脂联素及血管细胞黏附分子-1表达水平与不良妊娠结局显著相关。Objective:To investigate the relationship between visfatin,adiponectin(APN),and vascular cell adhesion molecule-1(VCAM-1)expression levels in pregnant women with metabolic syndrome(GMS)and the outcome of high-risk pregnancy.Methods:A retrospective study of 60 pregnant women with gestational metabolic syndrome who were admitted to the obstetrics department of Tangshan Maternal and Child Health Hospital from June 2016 to June 2018 was admitted as the observe group and 60 healthy pregnant women involved in the control group.The expression level of visfatin,APN and VCAM1 in maternal serum and placenta tissue and the related metabolic parameters were detected.The pregnancy outcomes of the two groups were compared and the correlation between each index and adverse pregnancy outcome was analyzed.Results:The expression level of visfatin and VCAM-1 in serum and placenta tissue of the observe group(pregnant women with gestational metabolic syndrome)were significantly higher than those of the control group(P<0.05),and APN was significantly lower than the control group(P<0.05).The serum LDL-C,TC,TG,FPG and HOMA-IR in the observe group were significantly higher than those in the control group(P<0.05),and the HDL-C was significantly lower than the control group(P<0.05).The incidence of giant children and premature infants was significantly higher than that of the control group(P<0.05).The visfatin and VCAM-1 were significantly positively correlated with the occurrence of adverse pregnancy outcomes in pregnant women with gestational metabolic syndrome,and APN was significantly negatively correlated.Conclusion:The expression level of visfatin,APN and VCAM-1 in pregnant women with gestational metabolic syndrome was significantly correlated with adverse pregnancy outcomes.
