摘  要:目的探讨中孕早期超声四维时空关联成像(STIC)技术诊断胎儿心脏畸形(CM)的临床价值。方法选取2014年6月至2018年6月疑似胎儿CM孕妇220例,于孕13-18+6w的中孕早期、孕19-27+6w的中孕晚期,所有孕妇均给予超声四维STIC技术检查,以产后病理或超声检查为金标准。结果产后病理或超声结果显示,220例孕妇中,胎儿CM有40例(18.18%);在超声四维STIC技术诊断胎儿CM敏感度、特异度、准确度中,中孕早期为82.50%、91.11%、89.55%,中孕晚期为87.50%、92.78%、91.82%,中孕早期联合晚期为97.50%、97.22%、97.27%,中孕早期联合晚期明显高于中孕早期、中孕晚期,有统计学差异(P<0.05)。结论超声四维STIC技术是诊断胎儿CM的重要方法,且中孕早期、晚期联合可有效提高胎儿CM的诊断价值。Objective:To discuss the clinical value of four-dimensional space-time correlation imaging(STIC)of ultrasound in the diagnosis of fetal cardiac malformation(CM)in early second trimester.Methods:220 pregnant women with suspected fetal CM were selected from June 2014 to June 2018.All pregnant women were given four-dimensional STIC of ultrasound examination in the early second trimester(13-18+6 weeks of pregnancy)and the late second trimester(19-27+6 weeks of pregnancy),and the postpartum pathology or ultrasonography were the gold standard.Results:The Postpartum pathological or ultrasonography results showed that,there were 40(18.18%)of 220 pregnant women had CM in their infants.In the sensitivity,specificity,accuracy of four-dimensional STIC in the diagnosis of fetal CM,the early second trimester were 82.50%,91.11%,89.55%,the late second trimester were 87.50%,92.78%,91.82%,the early combined late second trimester were 97.50%,97.27%,97.27%,the early combined late second trimester were significantly higher than those in the early second trimester and late second trimester,the difference was statistically significant(P<0.05).Conclusions:Four-dimensional STIC technique of ultrasound is the important method for the diagnosis of fetal CM,and the combination of early and late second trimester can effectively improve the diagnostic value of fetal CM.
