良性前列腺增生汽化电切术后膀胱痉挛的影响因素分析及预防建议

摘　　要：目的探讨良性前列腺增生(BPH)汽化电切术后膀胱痉挛的影响因素,并提出预防建议。方法选取常德市第一人民医院2017年1月-2018年10月收治的BPH患者185例,将术后出现膀胱痉挛的患者分为发生组22例,未出现膀胱痉挛分为未发生组163例。两组均行经尿道前列腺汽化电切术(TUEVAP),回顾性分析两组一般资料,术后膀胱痉挛的影响因素分析采用多因素Logistic回归分析。结果发生组膀胱冲洗液温度<34℃、膀胱冲洗液速度<60滴/min或>80滴/min、导尿管气囊注水量>40 ml、术后导尿管留置时间>7 d占比高于未发生组(P<0.05);两组年龄、前列腺增生体积、合并基础疾病、手术时间、导尿管型号比较,差异无统计学意义(P>0.05)。多因素Logistic回归分析结果显示,膀胱冲洗液温度<34℃、膀胱冲洗液速度<60滴/min或>80滴/min、导尿管气囊注水量>40 ml、术后导尿管留置时间>7 d是导致术后膀胱痉挛的独立危险因素(P<0.05)。结论 BPH汽化电切术后膀胱痉挛的发生风险较大,膀胱冲洗液温度<34℃、膀胱冲洗液速度<60滴/min或>80滴/min、导尿管气囊注水量>40 ml、术后导尿管留置时间>7 d属于危险因素,应制定有效的预防建议。Objective To explore the influencing factors of bladder spasm after vaporization electroresection of BPH,and to put forward the preventive suggestions.Methods A total of 185 cases of patients with BPH were selected from January 2017 to October 2018 in the First People’s Hospital of Changde Ctiy.The patients with postoperative bladder spasm were divided into occurrence group(n=22)and non occurrence group(n=163).TUEVAP was performed in both groups.The general data of the two groups were analyzed retrospectively.The influencing factors of postoperative bladder spasm were analyzed by multivariate Logistic regression analysis.Results The proportion of the temperature of bladder washing liquid<34 ℃,the velocity of bladder washing liquid<60 drops/min or>80 drops/min,the volume of water injected into the catheter bag>40 ml and the indwelling time of catheter>7 days in occurrence group were higher than the non occurrence group(P<0.05).There was no significant difference in age,volume of BPH,basic diseases,operation time and catheter type between the two groups(P>0.05).The results of multivariate Logistic regression analysis showed that the temperature of bladder washing liquid<34 ℃,the velocity of bladder washing liquid<60 drops/min or>80 drops/min,the volume of water injected into the catheter bag>40 ml and the indwelling time of catheter>7 days were the independent risk factors for postoperative bladder spasm(P<0.05).Conclusion The risk of bladder spasm after BPH electrovaporization is high.The temperature of bladder washing liquid<34 ℃,the velocity of bladder washing liquid<60 drops/min or>80 drops/min,the volume of water injected into the catheter bag>40 ml and the indwelling time of catheter>7 days were risk factors,effective prevention suggestions should be made.
