摘　　要：目的系统剖析沈阳市第四人民医院不合理使用碳青霉烯类抗菌药物的类型,以提高碳青霉烯类抗菌药物的合理使用水平。方法选取2018年1月至2018年12月沈阳市第四人民医院使用碳青霉烯类抗菌药物的全部病例376份作为研究对象,对不合理使用类型进行帕累托图分析,明确不合理使用类型并提出整改意见。结果碳青霉烯类使用存在不合理情况共49例,占全部病例的13.03%。帕累托图分析结果显示不合理用药类型中主要因素有抗菌药物用药起点过高、用法用量不适宜、使用碳青霉烯类未会诊、用药疗程过长;次要因素有使用碳青霉烯类前未送检;一般因素为联合用药不适宜、越级使用且在24 h内未补办手续、围手术期预防用药选择碳青霉烯类。结论利用帕累托图分析碳青霉烯类不合理使用类型并进行干预,可促进碳青霉烯类抗菌药物合理使用。Objective In order to improve the rational use of carbapenems, the unreasonable use of carbapenems in the Fourth People’s Hospital of Shenyang was analyzed. Methods A total of 376 cases of using carbapenems in Shenyang No.4 People’s Hospital from January 2018 to December 2018 were selected as research objects, and Pareto chart analysis was carried out for the types of unreasonable use, and the types of unreasonable use were identified and rectification suggestions were put forward. Results There were 49 cases of unreasonable use of carbapenems, accounting for 13.03% of all cases. Pareto diagram analysis showed that the primary factors of irrational drug use were high starting point of antimicrobial drugs, inappropriate usage and dosage, no consultation before use of carbapenems and long course of use of carbapenems;the secondary factors were that the secondary factor was that carbapenems were not tested before use;the general factors were that combination drug use was not suitable, over-grade use and no supplementary procedures within 24 hours and carbapenems were used for perioperative prophylaxis. Conclusion Using Pareto Diagram to analyze the unreasonable use types of carbapenems and intervene to promote the rational use of carbapenems.
