摘  要:目的建立控制性皮层撞击大鼠脑损伤模型,运用动态增强磁共振成像(DCE-MRI)技术与血清S100β蛋白检测,监测创伤性脑损伤(TBI)后血脑屏障(BBB)通透性的动态变化规律。方法成年SD大鼠60只,随机分为损伤组和假手术组,采用控制性皮层撞击法建立TBI模型;在建模后不同时间点(3 h,1、3、7、14天)行MRI检查和血清S100β检测,每组在每时间点各6只。DCE-MRI图像经处理后,得到大脑创伤层面的各灌注参数的相对值(rKtrans、rVe、rKep、rVp),随后与血清S100β浓度、脑组织内伊文思蓝(EB)漏出量(作为病理学"金标准")相比较,观察创伤后各时间点BBB通透性变化规律。结果创伤后各时间点的DCE-MRI灌注参数相对值(rKtrans)、血清S100β蛋白水平、脑组织EB渗漏量,在损伤组/假手术组之间的差异均有统计学意义(P<0.05)。其中,损伤组rKtrans自损伤后3 h轻度升高,1天明显升高,3天达到峰值,随后逐渐下降,14天时仍有轻度升高,呈单向性变化;血清S100β蛋白在伤后3 h轻度增加,持续至1、3天时迅速上升后下降并维持较低的水平。相关分析显示rKtrans、rVe值与EB漏出量之间均呈正相关(r=0.712,P<0.01;r=0.544,P<0.01);血清S100β蛋白值与EB漏出量之间呈正相关(r=0.686,P<0.01)。结论创伤性脑损伤后BBB通透性的变化有一定规律。与血清S100β蛋白相比,DCE-MRI定量参数Ktrans能更加准确的评估创伤性脑损伤后BBB通透性的变化,或许能成为TBI研究的有力工具。Objective A rat model of brain injury induced by controlled cortical impact was established. The dynamic changes of blood-brain barrier permeability after traumatic brain injury were monitored by dynamic enhanced magnetic resonance imaging and serum S100βprotein detection. Methods Sixty adult SD rats were randomly divided into injury group and sham operation group. The model of traumatic brain injury was established by controlled cortical impact method.MRI examination and serum S100βdetection were performed at different time points(3 h,1 d,3 d,7 d,14 d) after modeling,and each group was 6 at each time point. After DCE-MRI images were processed,the perfusion parameters of the brain trauma level were calculated-relative Ktrans(rKtrans),relative Ve(rVe),relative Kep(rKep),and relative Vp(rVp).Subsequently,compared with serum S100β concentration and Evans blue leakage,the change of BBB permeability at each time point after trauma was observed. Results There were significant differences in EB extravasation,DCE-MRI parameters(rKtransvalues) and serum S100β protein expression between the injury group and the sham group at each time point(P<0.05). rKtrans showed a uniphasic change,and it increased slightly at 3 h after injury,and increased significantly at 1 d,peaked at 3 d,then gradually decreased,and remained at a low level at 14 d. Serum S100β protein increased slightly at 3 h after injury and lasted for 1 d. After 3 d,it rapidly increased and then decreased to a lower level. Correlation analysis showed a positive correlation between rKtrans value and EB extravasation(r=0.712,P<0.01);there was a positive correlation between rVe value and EB extravasation(r=0.544,P<0.01);serum S100βprotein value and EB extravasation There was a positive correlation between the quantities(r=0.686,P<0.01). Conclusion There is a certain regularity in the change of BBB permeability after traumatic brain injury. Compared with serum S100β protein,Ktranscould more accurately assess the changes of BBB permeability after traumatic brain
