摘 要:目的应用剪切波弹性成像(SWE)监控离体猪肝组织硬度值随温度变化情况,探讨SWE在监控光热治疗中的作用。方法以水浴法对新鲜离体猪肝进行加热。应用SWE检测肝组织自35℃升温至85℃时的组织硬度值,以及肝组织维持一定温度10 min前、后组织硬度值变化;并与病理组织学进行对照分析。结果40℃~50℃时肝组织硬度值分别与35℃时比较差异均无统计学意义;55℃~85℃时肝组织硬度值分别与35℃组比较差异均有统计学意义(均P<0.05);35℃~50℃肝组织硬度值两两比较差异均无统计学意义;55℃~85℃肝组织硬度值分别与35℃~50℃时两两比较,差异均有统计学意义(均P<0.05)。肝组织病理图显示,随温度升高肝窦变窄,细胞逐渐水肿直至破裂。50℃~85℃时肝组织硬度值与连续升温实验中对应温度比较差异均无统计学意义;50℃~85℃时肝组织维持温度10 min前、后组织硬度值比较差异均无统计学意义;85℃时肝组织维持温度恒定10 min前、后组织硬度值比较差异有统计学意义[(119.31±2.91)kPa vs.(158.64±1.49)kPa,P<0.05]。肝组织病理图显示,50℃~85℃维持温度10 min前、后肝组织无明显变化。结论应用SWE可精确评价光热治疗时离体猪肝组织硬度随温度变化情况。ObjectiveTo study the value of porcine liver temperature changes by shear wave elastography(SWE)inphotothermal ablation.MethodsFresh porcine livers were heated with water bath method.The stiffness of liver tissue,withtemperature rising from 35℃to 85℃and maintaining at a certain level for 10 min,was respectively tested by SWE,andmeanwhile,compared with histopathology respectively.ResultsThere were no significant difference between the stiffness ofliver tissue at 40℃~50℃and 35℃,and there were significant difference between the stiffness of liver tissue at 55℃~85℃and35℃(all P<0.05).There were no significant difference of liver tissue hardness at 35℃~50℃in pairwise comparison,and therewere significant difference between the liver tissue hardness at 55℃~85℃and 35℃~50℃,respectively(all P<0.05).Histopathology showed that the hepatic sinus structure narrowed with the increase of temperature,and the cells were graduallyedema and rupture.There were no significant difference between liver tissue hardness at 50℃~85℃and the correspondingtemperature in the continuous heating experiment.There were no significant difference between the stiffness of liver tissue at50℃~85℃and that at the specified temperature of 10 min after maintenance,the difference in tissue stiffness values before and afterthe liver tissue was maintained at a constant temperature for 10 min at 85℃was statistically significant[(119.31±2.91)k Pa vs.(158.64±1.49)k Pa,P<0.05].The liver histopathology showed that there were no significant changes in the liver tissue between50℃~85℃and the temperature maintained for 10 min.ConclusionSWE can accurately evaluate the temperature changes ofporcine liver tissue during photothermal therapy.
