三维斑点追踪超声心动图评价高尿酸血症患者左心室功能的价值探讨

摘　　要：目的探讨三维斑点追踪超声心动图(3D-STE)在评价高尿酸血症患者左心室功能中的价值。方法选取我院80例高尿酸血症患者(高尿酸血症组)和40例健康志愿者(对照组),检测并比较两组实验室检查结果、常规超声参数左室舒张末期容积、左室收缩末期容积、每搏量(SV)、左室射血分数,以及3D-STE参数整体纵向应变(GLS)和整体圆周应变(GCS);分析高尿酸血症患者左心室3D-STE参数与尿酸水平之间的相关性。结果高尿酸血症组与健康对照组C-反应蛋白、谷丙转氨酶、谷草转氨酶、尿素氮、尿酸、肌酐比较差异均有统计学意义(均P<0.05)。高尿酸血症组SV、GLS、GCS较对照组显著降低[(54.71±9.60)ml vs.(61.92±14.40)ml、(-20.51±4.00)%vs.(-23.20±4.00)%、(-31.30±5.00)%vs.(-35.65±2.50)%],差异均有统计学意义(均P<0.05)。GCS与尿酸水平呈弱相关(r=0.269,P=0.039)。多重线性回归分析显示,尿酸对GCS的影响最大(标准偏回归系数=0.743,P=0.002)。结论3D-STE技术是检测高尿酸血症患者左心室功能的可靠方法,高尿酸血症是左心功能不全的独立危险因素。ObjectiveTo assess the value of the left ventricular(LV)function evaluated by three-dimensional speckle tracking echocardiography(3 D-STE)in patients with hyperuricemia.MethodsForty healthy controls(control group)and 80 patients with hyperuricemia(hyperuricemia group)were collected.Laboratory tests,conventional ultrasonic parameters(LVEDV,LVESV,SV,LVEF)and 3 D-STE parameters,including global longitudinal strain(GLS)and global circumferential strain(GCS)were compared between two groups,and the correlation between left ventricular 3 D-STE parameters and uric acid levels in patients with hyperuricemia were analyzed.ResultsThere were statistically significant differences in C-reactive protein,alanine transaminase,aspartate transaminase,blood urea nitrogen,serum creatinine,uric acid and creatinine between the hyperuricemia group and the control group(all P<0.05).Compared with control group,the SV,GLS and GCS were significantly decreased in hyperuricemia group[(54.71±9.60)ml vs.(61.92±14.40)ml,(-20.51±4.00)%vs.(-23.20±4.00)%,(-31.30±5.00)%vs.(-35.65±2.50)%,all P<0.05].Furthermore,GCS was significantly correlated with the serum uric acid(r=0.269,P=0.039).Multiple linear regression analysis showed that uric acid had the greatest effect on GCS(standard partial regression coefficient=0.743,P=0.002).Conclusion3 D-STE is a reliable method to detect LV function in patients with hyperuricemia,which is an independent risk factor for LV dysfunction.
