摘　　要：目的研究阻塞性呼吸睡眠暂停综合征(OSAS)患者肺动脉硬化度(PAS)与右心室舒张、收缩功能之间的相关性。资料与方法选取符合OSAS诊断标准的60例患者作为OSAS组,选取同期31例健康成人作为对照组。所有受试者均进行常规超声心动图检查,采用sRT-3DE技术评估OSAS右心室收缩功能。通过呼吸暂停低通气指数(AHI)判断OSAS患者的疾病严重程度,分析PAS及AHI与右心室舒张、收缩指标的相关性。结果OSAH组与对照组患者年龄、性别、心率、体重指数、血压差异均无统计学意义(P>0.05)。OSAH组患者AHI、鼾声指数和最低氧饱和度均高于对照组,差异有统计学意义(P<0.05)。OSAS组患者PAS与AHI呈显著正相关(r=0.722,P=0.000),与tE、tE/tA、tE'、tA'所代表右心室舒张功能指数呈显著负相关(r=-0.331,P=0.010;r=-0.341,P=0.008;r=-0.352,P=0.006;r=-0.521,P=0.000),与tA呈正相关(r=0.341,P=0.008),与右心室每搏量、右心室射血分数、三尖瓣环平面收缩偏移、tSa所代表右心室收缩功能指数呈负相关(r=-0.176,P=0.180;r=-0.602,P=0.000;r=-0.347,P=0.007;r=-0.497,P=0.000),与右心室舒张末期容积、右心室收缩末期容积、肺动脉收缩压、平均肺动脉压呈正相关(r=0.408,P=0.001;r=0.374,P=0.003;r=0.560,P=0.000;r=0.867、P=0.000)。结论OSAS患者右心室舒张及收缩功能随疾病严重程度的增加逐渐降低。作为新发现的一项多普勒超声参数,PAS与AHI呈正相关,并与OSAHS患者右心室收缩和舒张功能相关。Purpose To investigate the correlation between pulmonary artery stiffness(PAS)and right ventricular diastolic and systolic function in patients with obstructive sleep apnea syndrome(OSAS).Materials and Methods Sixty patients confirmed as OSAS were selected,and 31 healthy adults were selected as the control group.All patients underwent routine echocardiography,and right ventricular systolic function of OSAS was assessed using sRT-3DE technique.The apnea hypopnea index(AHI)was used to judge the severity of disease in OSAS patients,the correlations between PAS and AHI as well as right ventricular diastolic,systolic indices were analyzed.Results There were no significant differences in age,gender,heart rate,BMI and blood pressure between the OSAHS group and the control group(P>0.05).The AHI,snoring index and minimum oxygen saturation of the OSAHS group were statistically higher than those of the control group(P<0.05).There was a significant positive correlation between PAS and AHI in the OSAS group(r=0.722,P=0.000).PAS in the OSAS group showed significant negative correlation to right ventricular diastolic indices represented by tE,tE/tA,tE'and tA'(r=-0.331,P=0.010;r=-0.341,P=0.008;r=-0.352,P=0.006;r=-0.521,P=0.000),positive correlation to tA(r=0.341,P=0.008),significant negative correlation to right ventricular systolic indices represented by right ventricular stroke volume,right ventricular ejection fraction,tricuspid annular plane systolic excursion and tSa(r=-0.176,P=0.180;r=-0.602,P=0.000;r=-0.347,P=0.007;r=-0.497,P=0.000),and significant positive correlation to right ventricular end diastolic volume,right ventricular end systolic volume,systolic pulmonary artery pressure and mean pulmonary artery pressure(r=0.408,P=0.001;r=0.374,P=0.003;r=0.560,P=0.000;r=0.867,P=0.000).Conclusion The right ventricular diastolic and systolic functions in OSAHS patients decrease with the increased severity of the disease.As a new Doppler ultrasound parameter index,PAS is positively correlated to AHI and also shows correlation to
