摘 要:目的研究血清成纤维细胞生长因子23(FGF23)、胰岛素样生长因子-1(IGF-1)、Ⅰ型前胶原蛋白氨基端前肽(PINP)联合检测对绝经期妇女骨质疏松的辅助诊断价值。方法选取2017年1月至2019年6月经我院收治的243例绝经后女性临床资料做回顾性分析。将其根据骨密度测量值分为研究组(骨密度异常,n=131)和对照组(骨密度正常,n=112)。比较两组血清FGF23、PINP、IGF-1水平。将研究组患者根据骨密度测量将其分为骨质疏松组(n=75)、骨量减少组(n=56)。比较两组血清FGF23、PINP、IGF-1水平,并收集所有骨质疏松患者骨密度相关指标,多因素Logistic回归分析影响患者危险因素,分析血清FGF23、PINP、IGF-1联合检测对绝经期妇女骨质疏松的辅助诊断价值。结果研究组患者血清FGF23、PINP水平高于对照组(P<0.05),且IGF-1水平低于对照组(P<0.05);骨质疏松组患者血清FGF23、PINP水平高于骨量减少组(P<0.05),且IGF-1水平低于骨量减少组(P<0.05);骨质疏松与骨量减少的疏松部位、骨量丢失类型比较,差异有统计学意义(P<0.05);血清FGF23、PINP、IGF-1联合检测对绝经期妇女骨质疏松的诊断价值低于血清FGF23、PINP、IGF-1指标、骨密度联合检测(P<0.05),但高于血清FGF23、PINP、IGF-1指标单项检测(P<0.05)。结论血清FGF23、PINP、IGF-1水平变化与绝经期妇女骨质疏松病情发展相关,联合三者对绝经期妇女骨质疏松患者有一定辅助诊断价值,为早期监测骨量,制定和调整治疗方案提供参考。Objective To investigate the value of combined detection of serum fibroblast growth factor 23(FGF23),insulin growth factor 1(IGF-1)and amino terminal pro-peptide of type I procollagen(PINP)in the diagnosis of osteoporosis in menopausal women.Methods Clinical data of 243 postmenopausal women admitted to our hospital from January,2017 to June,2019 were retrospectively analyzed.Women patients were divided into the study group(n=131)and control group(n=112)according to bone mineral density.The study group was further divided into two subgroups:the osteoporosis group(n=75)and osteopenia group(n=56).The serum FGF23,PINP and IGF-1 levels were compared among groups.Multivariate logistic regression analysis was used to analyze the risk factors.The value of combined detection of serum FGF23,PINP and IGF-1 in the diagnosis of osteoporosis in menopausal women was evaluated.Results The serum levels of FGF23 and PINP in the study group were higher than those in the control group(P<0.05),and the level of IGF-1 was lower than that in the control group(P<0.05).The levels of serum FGF23 and PINP in the osteoporosis group were higher than those in the osteopenia group(P<0.05),and the IGF-1 level was lower than that in the osteopenia group(P<0.05).The location of osteoporosis and the type of bone loss showed significant difference between the osteoporosis group and osteopenia group(P<0.05).The diagnostic value of combined detection of serum FGF23,PINP and IGF-1 for osteoporosis in menopausal women was lower than that of combined detection of serum FGF23,PINP,IGF-1 and bone mineral density(P<0.05),but higher than that of single detection of serum FGF23,PINP or IGF-1(P<0.05).Conclusion The changes of serum FGF23,PINP,and IGF-1 levels are associated with the development of osteoporosis in menopausal women,and the combined detection of these three has a high diagnostic value,which can provide a reference for the early diagnosis.
