摘  要:目的采用PACS系统及Infinitt软件测量可注射螺钉周围聚甲基丙烯酸甲酯(polymethylmethacrylate,PMMA)骨水泥的分布规律。方法将骨质疏松人工骨模块采用随机数字表法随机均分为A组(180°-4孔螺钉)、B组(180°-6孔螺钉)、C组(180°-间隔4孔螺钉),每组又随机均分为低、中、高3个剂量组(分别注入1.0、1.5、2.0ml骨水泥)。采用相同方法制备钉道后,A、B、C组分别拧入相应的可注射螺钉并注射不同剂量骨水泥。通过X线检查观察螺钉及骨水泥分布情况,采用PACS系统及Infinitt软件测量正位上骨水泥分布的最高点和最低点到钉尖的距离,取二者的平均值。结果 A组螺钉近端侧孔到钉尖的距离为(11.98±0.13)mm,明显小于B组的(17.36±0.13)mm和C组的(17.48±0.10)mm,差异有显著性(P<0.05),而B组和C组比较差异无显著性(P=0.055)。分别注射1.0、1.5、2.0ml骨水泥时,A组骨水泥距钉尖的距离明显小于B组和C组,差异有显著性(P<0.05),而B组和C组比较差异无显著性(P>0.05)。结论 180°-4孔螺钉骨水泥的分布与180°-6孔螺钉和180°-间隔4孔螺钉存在显著差异,而后两者的骨水泥分布较为一致。Objective To evaluate the feasibility of using PACS system and Infinitt software to measure PMMA distribution parameters around screws. Method Synthetic bone blocks were randomly divided into groups A, B, C with different kinds of screws, and then blocks in each group were randomly divided into subgroups 1, 2, 3 with different volumes of PMMA. Pedicle screws of type A-C were directly inserted into vertebrae in groups A-C respectively, and then different volumes of PMMA(1.0, 1.5, 2.0 ml) were injected through screw into blocks in subgroups 1, 2, and 3 respectively. X-ray examination was performed to evaluate distribution of PMMA. The relevant parameters of PMMA distribution around the screw were measured by PACS system and Infinitt software. The distance between the highest point and the lowest point of the PMMA distribution in the positive position to the nail tip was measured, and the average of the two data was taken. Result The distance from the proximal side holes of the screw to the tip of the nail in the group A-C group was different. There were statistical differences between the three groups(P<0.05). The distance from the proximal side holes of the screw to the tip in group A were significantly smaller than those in groups B and C(P<0.05). There was no statistical difference between group B and group C(P=0.055). There were statistical differences between the distribution parameters of PMMA in three groups(P<0.05) when 1.0, 1.5, 2.0 ml PMMA were injected. The distances in group A were significantly smaller than those in groups B and C(P<0.05), while there was no statistical difference between group B and and group C(P>0.05). This result indicated that there was a significant difference between the distribution of PMMA in group A and group B and group C when the same dose of PMMA was injected, and there was no significant difference in the distribution of PMMA between group B and group C.
