摘  要:目的探讨力竭游泳诱导动脉粥样硬化斑块破裂及抗阻运动对动脉粥样硬化易损斑块形态和稳定性的影响。方法 30只ApoE-/-小鼠随机分3组,对照组(Ctl组)、力竭组(Exh组)、抗阻+力竭组(Res+Exh组),全程高脂喂养,全部小鼠行颈总动脉套管术,术后Exh组进行2周力竭游泳运动,Res+Exh组先进行8周抗阻运动后实施2周力竭游泳运动。比较实验前后小鼠体质量变化。取颈总动脉,石蜡切片,苏木素-伊红(HE)染色观察斑块大小、Masson染色观察胶原成分;冰冻切片,油红O染色观察脂质沉积;免疫组化检测巨噬细胞、平滑肌细胞在局部的表达。计算各组斑块的易损指数。取血清检测总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDL-C)浓度。结果实验前后3组小鼠体质量比较,Ctl组和Exh组增加大于Res+Exh组(P=0.009);3组TG、TC、LDL-C、HDL-C比较,Ctl组血脂水平高于Exh组和Res+Exh组,Res+Exh组低于Exh组,差异有统计学意义(P <0.05)。斑块大小、纤维帽厚度、易损指数3组比较,Res+Exh组面积较Ctl组和Exh组小,Exh组纤维帽厚度小于Ctl组和Res+Exh组,Res+Exh组斑块面积最小,纤维帽厚度较Exh组厚,易损指数Exh组明显高于Ctl组和Res+Exh组,Res+Exh组高于Ctl组,差异有统计学意义(P <0.05)。结论力竭游泳运动诱导动脉粥样硬化易损斑块破裂,抗阻运动减少易损斑块的破裂,起到稳定斑块的作用。Objective To investigate the effects of exhaustive swimming inducing atherosclerotic plaque rupture and resistance exercise on the morphology and stability of atherosclerotic vulnerable plaque. Methods Thirty ApoE-/-mice were randomly divided into 3 groups,control group(group Ctl),exhaustion group(group Exh),resistance combined exhaustion group(group Res+Exh). All mice were fed a high-fat diet throughout the course and underwent carotid cannulation. After te carotid cannulation,the postoperative group Exh performed exhaustive swimming 2 weeks,and the group Res+Exh performed 8 weeks of resistance exercise followed by 2 weeks of exhaustive swimming. The body weight change of the mice before and after the experiment was compared. Common carotid arteries were made into paraffin sections. Hematoxylin-eosin(HE)staining was to observe plaque size,Masson staining to observe collagen components;as for frozen sections,oil red O taining to observe lipid deposition. Local expression of smooth muscle cells and macrophages were detected by immunohistochemistry. Serum was measured for TG,TC,LDL-C,HLDL-C concentration by ELISA. Results Compared with the weight before the experiment,the weight increase in group Ctl and group Exh was more than that in group Res+Exh after that(P=0.009),which was statistically significant. The levels of TG,TC,LDL-C and HLDL-C in the three groups were higher than those in group Exh and group Res+Exh,and group Res+Exh was lower than group Exh(P<0.05).Comparing the plaque size,fiber cap thickness,and vulnerability index,the plaque area of group Res+Exh was smaller than the group Ctl and the group Exh;the fiber cap thickness of group Exh was smaller than the group Ctl and the group Res+Exh;and the group Res+Exh had the smallest plaque area;the thickness of the fiber cap was thicker than that of the group Exh;the vulnerability index of group Exh was significantly higher than the group Ctl and the group Res+Exh;and the group Res+Exh was higher than the group Ctl.
