疏血通注射液联合Fogarty导管取栓等治疗外科术后急性下肢深静脉血栓形成疗效观察

摘　　要：目的观察疏血通注射液联合Fogarty导管取栓等治疗外科术后急性下肢深静脉血栓形成(DVT)的临床疗效。方法将50例外科术后急性下肢DVT患者按照随机数字表法分为2组。2组均行下腔静脉滤器植入,对照组25例予低分子肝素钙抗凝+Fogarty导管顺向及逆向取栓治疗,治疗组25例在对照组基础上加用疏血通注射液经足背静脉滴注治疗。比较2组治疗前后双下肢周径差值、血凝指标[D-二聚体(D-D)、凝血酶原时间(PT)、纤维蛋白原(FIB)、活化部分凝血酶原时间(APTT)],并统计疗效。结果 2组手术成功率100%。治疗组总有效率96%,对照组总有效率80%,治疗组疗效优于对照组(P<0.05)。2组治疗后大腿、小腿周径差值均减小(P<0.05),且治疗组治疗后大腿、小腿周径差值均小于对照组(P<0.05)。2组治疗后D-D、FIB均降低(P<0.05),PT、APTT均延长(P<0.05);治疗组治疗后D-D、FIB均低于对照组(P<0.05),PT、APTT均高于对照组(P<0.05)。结论疏血通注射液联合Fogarty导管取栓等治疗外科术后急性下肢DVT是一种创伤小且安全有效的方法,尤其对于存在溶栓禁忌者更为适用。Objective To observe the effect of Shuxuetong injection combined with Fogarty catheter thrombectomy in the treatment of postoperative acute lower extremity deep vein thrombosis(DVT). Methods 50 patients with acute lower extremity DVT were randomly divided into two groups according to random number table method. Two groups were implanted with inferior vena cava filter. 25 cases in control group were treated by low molecular weight heparin calcium anticoagulation plus Fogarty catheter anterograde and reverse thrombectomy. 25 cases in treatment group were treated by Shuxuetong injection via dorsal vein of foot on the basis of the control group. The difference of peripheral diameter between two lower limbs, blood coagulation indexes [D-dimer(D-D), prothrombin time(PT), fibrinogen(FIB), activated partial thromboplastin time(APTT) ] were compared between the two groups before and after treatment, and the curative effect was statistically analyzed. Results The success rate of the two groups was 100%. The total effective rate was 96% in treatment group and 80% in control group. The clinical curative effect of the treatment group was better than that of the control group(P<0.05). After treatment, the difference of peripheral diameter of thigh and crus in the two groups were decreased(P<0.05), and the difference in the treatment group was smaller than that in the control group(P<0.05). After treatment, D-D and FIB were decreased in two groups(P<0.05), PT and APTT were prolonged(P<0.05). The D-D and Fib in the treatment group were lower than those in the control group(P<0.05), PT and APTT were higher than those in the control group(P<0.05). Conclusion Shuxuetong injection combined with Fogarty catheter thrombectomy is a safe and effective method for the treatment of postoperative acute lower extremity DVT, especially for the patients with thrombolysis taboo.
