摘 要:目的探讨超声清创联合湿性愈合技术治疗慢性创面的效果。方法选择2017年1月—2018年1月来我科进行慢性创面治疗的34例患者,采用数表法随机法将患者分为采用超声清创联合湿性愈合技术治疗的观察组(17例)和采用外科清创联合湿性愈合技术治疗的对照组(17例)。比较两组创面的疼痛程度、肉芽生长、细菌转阴率、创面缩小率及经济效益等情况。结果观察组与对照组首次清创时疼痛程度经卡方检验,差异具有统计学意义,观察组优于对照组(P<0.05)。两组创面肉芽生长评分经t检验,差异具有统计学意义(P<0.05),观察组优于对照组。观察组创面细菌转阴率100%,明显优于对照组创面细菌转阴率70.6%。两组创面愈合面积经t检验,差异具有统计学意义(P<0.05),观察组创面面积缩小明显。两组使用经济费用比较经t检验,差异具有统计学意义(P<0.05),观察组费用更为经济。结论超声清创联合湿性愈合技术能起到清创彻底、引流充分、维持创面湿性平衡,减轻清创时的痛苦,减轻创面感染和改善血运,刺激肉芽组织快速生长,缩短创面治愈时间,减少患者经济花销,达到明显促进创面修复的效果。Objective To explore the efficacy of ultrasonic debridement combined with wet healing in the treatment of chronic refractory wounds.Methods A total of 34 patients who came to our department from January 2017 to January 2018 for chronic difficult wound healing treatment were selected,and the patients were randomly divided into the observation group(17 cases)using ultrasonic debridement combined with wet healing technology and the control group(17 cases)using surgical debridement combined with wet healing technology.The pain degree,granulation growth,bacterial conversion rate,wound reduction rate and economic benefit of the two groups were compared.Results The wound pain degree of the observation group and the control group was tested by chi-square test(P<0.05),and the difference was statistically significant.The growth of wound granulation in the observation group and the control group was evaluated by t test(P<0.05).The difference was statistically significant,and the observation group was superior to the control group.The negative rate of wound bacteria in the observation group was 100%,which was obviously better than that of the control group(70.6%).The wound healing of the two groups was examined by t test(P<0.05),and the difference was statistically significant.The comparison of costs between the two groups showed significant difference through t test,(P<0.05),and the difference was statistically significant.In terms of mean value,the cost of the observation group was lower than that of the control group.Conclusion Ultrasonic debridement combined wet healing technology fully,maintaining wound debridement completely,drainage moisture balance,relieve the suffering of the debridement,dressing,reduce the wound infection and improve the blood supply,stimulating growth of granulation tissue quickly,shorten the wound healing time,reduce the patients'economic costs,in order to effect significantly promote wound repair.
