摘　　要：目的比较术前穿刺活检和术中冰冻病理诊断乳腺导管原位癌伴微浸润(DCIS-Mi)的准确性。方法回顾性分析116例乳腺DCIS-Mi患者(肿瘤直径均>2.0 cm)的临床资料,将行术前穿刺活检的51例患者作为观察组,行术中冰冻病理的65例患者作为对照组。以术后病理诊断结果为金标准,比较两种方法的诊断乳腺DCIS-Mi效能。结果观察组诊断乳腺DCIS-Mi的敏感度为82.5%(33/40),特异度为81.8%(9/11),一致率为82.4%(42/51),Youden指数为0.825;对照组诊断乳腺DCIS-Mi的敏感度为45.8%(22/48),特异度为64.7%(11/17),一致率为50.8%(33/65),Youden指数为0.336。观察组诊断乳腺DCIS-Mi的一致率高于对照组(P<0.05)。结论术前穿刺活检诊断乳腺DCIS-Mi的准确性优于术中冰冻病理诊断。Objective To compare the accuracy between preoperative puncture biopsy and intraoperative frozen section pathology for the diagnosis of breast ductal carcinoma in situ with microinvasion(DCIS-Mi).Methods The clinical data of 116 breast DCIS-Mi patients(tumor diameter >20 cm) were retrospectively analyzed.Fifty-one patients receiving preoperative puncture biopsy were enrolled as observation group,and 65 patients receiving intraoperative frozen section pathology as control group.With the result of postoperative pathology as the golden standard,the diagnostic efficiency for breast DCIS-Mi was compared between the two methods.Results For diagnosing breast DCIS-Mi,the sensitivity,specificity,concordance rate,and Youden index were 82.5%(33/40),81.8%(9/11),82.4%(42/51) and 0.825 respectively in the observation group,whereas were 45.8%(22/48),64.7%(11/17),50.8%(33/65) and 0.336 respectively in the control group.The concordance rate in the diagnosis of breast DCIS-Mi was higher in the observation group than that in the control group(P<0.05).Conclusion The accuracy of preoperative puncture biopsy in the diagnosis of breast DCIS-Mi is superior to that of intraoperative frozen section pathology.
