摘　　要：目的 通过分析老年血液透析患者发生导管相关感染的危险因素,为预防和治疗提供理论依据。方法 选取2010年3月~2018年9月新疆医科大学附属中医医院收治的458例接受静脉置管进行血液透析的老年患者,分析发生医院感染的病原菌、感染部位及其相关危险因素。结果 共有72例患者发生医院感染,发生率15.7%;共检出81株病原菌;患者感染涉及多个系统,以呼吸系统为主,占43.0%。年龄、体质量指数、穿刺位置、置管时间、糖尿病肾病、白细胞计数、血红蛋白、血清白蛋白和超敏C-反应蛋白是引起感染的危险因素(P<0.05)。结论 老年血液透析患者存在导管相关感染的危险因素,应有针对性的防治。Objective To analyze the risk factors of catheter-related infection in elderly hemodialysis patients and provide theoretical basis for prevention and treatment.Methods 458 elderly patients who received hemodialysis by intravenous catheterization in Affiliated Hospital of traditional Chinese medicine of Xinjiang Medical University from March 2010 to September 2018,were enrolled to analyze the pathogenic bacteria and infection sites of the patients with nosocomial infection.The pathogenic bacterium and infection sites were analyzed.The risk factors were compared with uninfected patients.Results There were 72 patients with nosocomial infection,the incidence was 15.7%;81 strains of pathogenic bacteria were detected;patients with nosocomial infection involved multiple systems,mainly respiratory system,accounting for 43.0%.Age,body mass index,puncture site,catheter indwelling time,diabetic nephropathy,lencocyte count,hemoglobin,blood serum albumn and high sensitive C-reactive protein were risk factors for infections,there were significance different between them(P<0.05).Conclusion There are risk factors of catheter-related infection in elderly hemodialysis patients,which should be targeted prevention and treatment.
