摘 要:目的 研究依帕司他应用于早期糖尿病视网膜病变对血清丙二醛(MDA)、超氧化物歧化酶(SOD)、总抗氧化能力(TAC)水平及其内皮功能保护。方法 选取2016年6月~2018年8月在青岛市第八人民医院接受治疗的126例早期糖尿病视网膜患者,按照随机数表法分为观察组(63例)和对照组(63例)。对照组使用前列地尔注射液治疗,观察组在对照组基础上应用依帕司他辅助治疗。观察两组患者治疗后临床疗效;分析两组治疗前后血清MDA、SOD、TAC、糖化血红蛋白(HbA1c)水平变化和内皮功能各项参数变化;以及观察两组患者在治疗期间和治疗后不良反应发生情况。结果 治疗后,观察组总有效率(87.30%)明显高于对照组(73.02%),差异有统计学意义(P<0.05);观察者MDA水平明显低于对照组[(9.16±2.12)mol/L vs.(12.06±3.02)mol/L],SOD、TAC水平明显高于对照组[(94.16±9.88)U/L vs.(82.96±10.02)U/L;(20.98±3.88)Ku/L vs.(19.22±4.12)Ku/L],差异有统计学意义(P<0.05);观察组内皮功能参数NO、vWF、TM水平明低于对照组,差异有统计学意义(P<0.05);观察组患者Hb A1c水平明显明显低于对照组(6.97±1.13)%vs.(7.56±1.50)%(P<0.05)。结论 依帕司他应用于早期糖尿病视网膜病变能够有效提高临床疗效,显著降低MDA水平及并提高SOD、TAC水平,提高内皮功能保护能力,调节患者血糖水平。Objective To study the efficacy of epalrestat in early diabetic retinopathy on serum malondialdehyde(MDA),superoxide dismutase(SOD),total antioxidant capacity(TAC)and endothelial function.Methods 126 patients of early diabetic retinopathy who received treatment from June 2016 to August 2018 were enrolled.According to the random number table method,the patients were divided into observation group(n=63)and control group(n=63).The control group was treated with alprostadil injection,and the observation group was treated with epalrestat adjuvant therapy on the basis of the control group.To observe the clinical effect after treatment;Serum levels of the two groups before and after treatment were analyzed.The changes of endothelial function parameters before and after treatment were compared between the two groups.The changes of fasting and 2h postprandial blood glucose in the two groups were analyzed.The adverse reactions of the two groups during and after treatment were observed.Results The total effective rate(87.30%)in the observation group was significantly higher than that in the control group(73.02%)(P<0.05).After treatment,MDA level of the observer was significantly less than that in the control group[(9.16±2.12)mol/L vs.(12.06±3.02)mol/L],SOD and TAC levels were significantly higher than that in the control group[(94.16±9.88)U/L vs.(82.96±10.02)U/L;(20.98±3.88)Ku/L vs.(19.22±4.12)Ku/L](P<0.05).After treatment,the levels of NO,vWF and TM in the observation group were lower than those in the control group(P<0.05).After treatment,HbA1c level in the observation group was significantly lower than that in the control group(6.97±1.13)%vs.(7.56±1.50)%(P<0.05).Conclusion The application of ipa in early diabetic retinopathy can effectively improve the clinical efficacy,significantly reduce the MDA level and increase the levels of SOD and TAC,improve the ability of endothelial function protection,and regulate the blood glucose level of patients.
