不同病因学类型急性胰腺炎胰腺坏死的临床特征分析

摘　　要：目的:探讨不同病因学类型急性胰腺炎(acute pancreatitis,AP)胰腺坏死的临床特征。方法:纳入2017年1月-2018年6月本院收治的AP患者135例,根据AP不同病因学类型划分为胆石症组(n=72)、酒精组(n=30)、高脂血症组(n=23)和其他病因组(n=10)4组,对比各组年龄、性别、身体质量指数(BMI)、基础疾病等人口学特征,通过分析CT影像学检查对比各组胰腺坏死发生比例、胰腺坏死发生部位、类型和程度,对比各组AP严重程度分级、器官衰竭发生情况和类型。结果:胆石症组患者年龄偏大、女性比例较高、合并基础疾病较多(P<0.05)。各组胰腺坏死发生比例、胰腺坏死发生部位、类型和程度比较,差异均无统计学意义(P>0.05);各组AP严重程度分级、器官衰竭发生情况比较,差异均无统计学意义(P>0.05)。结论:AP胰腺坏死发生比例、部位、类型和程度与AP病因学类型无关。Objective:To investigate the clinical features of pancreatic necrosis in patients with acute pancreatitis(AP)with different etiological types.Method:A series of 135 patients with AP were enrolled from January 2017 to June 2018.According to the different etiological types of AP,the patients were divided into four groups:gallstone group(n=72),alcohol group(n=30),hyperlipidemia group(n=23)and other etiological groups(n=10).The demographic characteristics of age,sex,body mass index(BMI),basic diseases in four groups were compared.The proportion,location,type and degree of pancreatic necrosis were compared by CT imaging,and the severity of AP,occurrence and type of organ failure were compared.Result:The patients in gallstone group were older,with a higher proportion of women and more complicated with underlying diseases(P<0.05).There were no significant differences in the proportion of pancreatic necrosis,location,type and degree of pancreatic necrosis among the groups(P>0.05).There were no significant differences in the severity of AP and the occurrence of organ failure among the groups(P>0.05).Conclusion:The proportion,location,type and degree of AP pancreatic necrosis are not related to the etiology of AP.
