大株红景天注射液联合低分子肝素对危重症机械通气患者凝血功能、呼吸机使用时间及预后影响的研究

摘　　要：目的探究大株红景天注射液联合低分子肝素对危重症机械通气患者凝血功能、呼吸机使用时间及预后影响的研究。方法选取我院2018年1~12月收治的危重症机械通气患者60例作为研究对象,按随机数字表法将其分为对照组(n=30)和观察组(n=30),对照组给予单用低分子肝素治疗,观察组给予大株红景天注射液联合低分子肝素治疗,比较两组患者治疗前后凝血功能指标,包括PT、INR、APTT、AT3、D-Ⅱ聚体、血小板的水平;比较两组患者深静脉血栓发生率及呼吸机使用时间;并观察两组患者预后包括住ICU时间、ICU病死率、28d生存率及住院总费用等预后情况。结果治疗后观察组各项凝血功能指标与对照组比较,差异有统计学意义(P<0.05);观察组患者深静脉血栓发生率较对照组降低,且其呼吸机使用时间短于对照组,差异有统计学意义(P<0.05);观察组患者ICU时间、及住院总费用均低于对照组,差异有统计学意义(P<0.05);而两组患者ICU病死率及28d生存率比较,差异无统计学意义(P>0.05)。结论大株红景天注射液联合低分子肝素治疗危重症机械通气患者可以改善凝血功能指标,缩短呼吸机使用时间,在一定程度上改善患者预后,值得临床推广应用。Objective To explore the research on effects of sofren injection combined with combined with low molecular weight heparin on coagulation function,using time of ventilator and prognosis in critically ill patients with mechanical ventilation.Methods 60 critically ill patients with mechanical ventilation who were admitted and treated in our hospital from January to December 2018 were selected as research objects.They were randomly divided into the control group(n=30)and the observation group(n=30)according to the random number table method.The control group was treated with low molecular weight heparin alone while the observation group was treated with sofren injection combined with combined with low molecular weight heparin.The coagulation function indexes including PT,INR,APTT,AT3,D-Ⅱaggregates and platelet levels of the two groups were compared before and after treatment.The incidence of deep venous thrombosis and the using time of ventilator were compared between the two groups.The prognosis of the two groups was observed,including ICU time,ICU mortality,28-day survival rate and total hospitalization expenses.Results After treatment,there were significantly differences in coagulation function indexes between the observation group and the control group(P<0.05).The incidence of deep vein thrombosis in the observation group was lower than that in the control group,and the using time of ventilator was shorter than that of the control group.The difference was statistically significant(P<0.05).The ICU time and t total hospitalization expenses of the observation group were lower than the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in ICU mortality and 28-day survival between the two groups(P>0.05).Conclusion Sofren injection combined with combined with low molecular weight heparin in treatment of critically ill patients with mechanical ventilation can improve coagulation function,shorten the using time of ventilator and improve the prognosis
