经鼻双相正压通气结合牛肺磷脂注射液治疗胎粪吸入综合征的效果及对患儿炎性因子和内皮功能的影响

摘　　要：目的探讨经鼻双相正压通气(N-BiPAP)结合牛肺磷脂注射液治疗胎粪吸入综合征的效果。方法将我院于2017年1月~2018年12月收治的82例胎粪吸入综合征患儿按照随机数字表法分为两组,对照组41例予以经鼻持续正压通气(NCPAP)结合牛肺磷脂注射液治疗,研究组41例予以(N-BiPAP)结合牛肺磷脂注射液治疗。比较两组患儿临床疗效、白细胞介素-8(IL-8)、白细胞介素-10(IL-10)、内皮素-1(ET-1)、一氧化氮(NO)水平、并发症发生情况以及行为神经评分量表(NBNA)评分。结果研究组临床总有效率为(92.68%)显著高于对照组(70.73%)(P<0.05)。研究组IL-8、ET-1水平低于对照组,IL-10、NO水平高于对照组(P<0.05)。出院3个月时研究组NBNA评分高于对照组(P<0.05)。结论N-BiPAP结合牛肺磷脂注射液治疗胎粪吸入综合征疗效确切,可减轻炎性反应及内皮功能损伤,促进患儿临床转归。Objective To explore the curative effects of nasal biphasic positive pressure ventilation(N-BiPAP)combined with bovine lung phospholipid injection in treatment of meconium aspiration syndrome.Methods 82 children with meconium aspiration syndrome who were admitted and treated in our hospital from January 2017 to December 2018 were randomly divided into two groups according to the random number method.41 patients in the control group were treated with nasal continuous positive airway pressure(NCPAP)combined with bovine lung phospholipid injection.41 patients in the study group were treated with nasal biphasic positive pressure ventilation(N-BiPAP)combined with bovine lung phospholipid injection.The clinical curative effects,interleukin-8(IL-8),interleukin-10(IL-10),endothelin-1(ET-1),nitric oxide(NO)levels,occurrence of complications and Neonatal behavioral neurological assessment(NBNA)scale score were compared between the two groups.Results The total clinical effective rate of the study group(92.68%)was significantly higher than that of the control group(70.73%)(P<0.05).The levels of IL-8 and ET-1 in the study group were lower than those in the control group while the levels of IL-10 and NO were higher than those in the control group(P<0.05).The NBNA score of the study group was higher than that of the control group on 3 months after discharge(P<0.05).Conclusion N-BiPAP combined with bovine lung phospholipid injection in treatment of meconium aspiration syndrome has accurate curative effects.It can reduce inflammatory reaction and endothelial function damage and promote clinical outcomes.
