LMWH与IPC在妇科术后预防VTE风险分级中危患者的临床疗效研究

摘　　要：目的探讨低分子肝素(LMWH)、机械性预防(IPC)在妇科术后VET风险分级中危患者的临床疗效。方法68例因妇科疾病在我院接受手术、风险分级为中危的患者,随机将患者分成对照组和观察组,每组各34例。给予对照组机械性预防方法,给予观察组低分子肝素抗凝方法。结果两组患者的术后VTE发生率为0。观察组凝血指标PT、APTT、TT凝血指标长于对照组,PLT、PSV水平低于对照组,差异有统计学意义(P<0.05)。HCT、大腿周径指标两组无明显差异(P>0.05)。对照组无并发症,观察组注射部分皮肤溃烂2例,注射时明显疼痛者6例,差异有统计学意义(P<0.05)。结论在妇科术后风险分级中危患者VET预防中LMWH抗凝、IPC方法临床效果无明显差异;LMWH抗凝可优化凝血指标,但有注射部位皮肤溃烂、出血等并发症。Objective To investigate the clinical efficacy of low molecular weight heparin(LMWH)and mechanical prophylaxis(IPC)in patients at risk of VET after gynecological surgery.Methods 68 patients with gynecological diseases who underwent surgery in our hospital were randomly divided into control group and observation group,34 cases in each group.The control group was given mechanical prevention method,and the observation group was given low molecular weight heparin anticoagulation method.Results The incidence of VTE was 0 in the two groups.The coagulation indexes of PT,APTT and TT in the observation group were longer than those in the control group,and the levels of PLT and PSV were lower than those in the control group(P<0.05).There was no significant difference in HCT and thigh circumference between the two groups(P>0.05).There were no complications in the control group,There were 2 cases of skin ulceration and 6 cases of obvious pain in the observation group(P<0.05).Conclusion There is no significant difference in the clinical effect of LMWH anticoagulation and IPC in the prevention of VET in patients at risk after gynecological surgery.LMWH anticoagulation can optimize coagulation parameters,but there are complications such as skin ulceration and bleeding at injection site.
